The FIND Programme is an innovative service design that transforms the way we are delivering specialist medical care in North East London (NEL). A tool has been developed to screen all results being processed by the Barts Health pathology laboratory, flagging patients against specific clinical indicators to identify those at risk of disease and allocate them into new pathways, aimed at optimising their treatment earlier in their clinical journey to improve outcomes and experience.
This postholder is responsible for the co-ordination of patient pathways for the Chronic Kidney Disease (CKD) workstream of the FIND Programme, ensuring smooth administration from the point of patient identification across all pathways.
The role involves maintaining accurate patient records, managing referrals, communicating with patients and clinical teams, and supporting service performance.
