We are looking to recruit an experienced care coordinator/key worker to work as a member of the multidisciplinary Community Locality Team. The role will require the post holder to:
To provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their well-being and recovery. This includes depot administration and regular monitoring of mental state.
To liaise with other agencies in planning and assessing programmes of care
Provide formal, on-going clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.
To undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required.
To complete full mental health assessments and complete Dialog+ as part of KPIs.
To produce holistic risk assessments
To regularly liaise with MDT and patients, including home visits, to ensure safety and wellbeing.
Main duties of the job
We have a duty of care towards our patients. We place them at the centre of the everything we do to ensure their safety and well-being. We treat with compassion, dignity and respect, embedding our trust values in everything we do. On a day to day basis, we take part in regular MDT meetings and morning safety huddles. We also promote clear communication with GPs, families and carers as well as local authority. We work together as a team, building a safe environment, where colleagues are respected and encouraged to take on an active role in MDT meetings.

To support service users on their recovery pathway
Work collaboratively with our partners carers. relatives. and other social inclusion
Identify risk and manage risk appropriately in the best interest of the service users and others.
To discuss cases at the MDT for professional input and support
To signpost service users to appropriate services as part of recovery pathways

Skills and Abilities
Essential
- A recognised professional qualification Nursing (RMN), Social Work, Occupational Therapy, or equivalent mental health qualification/registration.
- Experience working with adults with severe and enduring mental health conditions Including managing risk, working with complex presentations, and supporting individuals in the community.
- Ability to undertake comprehensive assessments Skilled in risk assessment, formulation, and care planning under CPA or equivalent frameworks.
- Strong risk management and clinical decision‑making skills Able to identify, escalate, and manage risks effectively, including safeguarding adults and children
- Ability to manage a clinical caseload independently Prioritising workload, maintaining accurate documentation, and supporting service users with recovery‑focused interventions.
- Effective communication and interpersonal skills Able to engage service users, carers, and work collaboratively with MDT colleagues, GPs, housing, crisis services, and partner agencies. Demonstrate ability to use initiative as appropriate.
- Proficiency with electronic clinical systems and record keeping Confident using systems such as RIO/EMIS and maintaining high‑quality, timely clinical notes in line with governance standards.
- Knowledge of relevant legislation and policy frameworks Including the Mental Health Act, Mental Capacity Act, safeguarding responsibilities, CPA, and NICE guidance.
Desirable
- Experience working within Community Mental Health Teams (CMHTs) Demonstrates understanding of community pathways, MDT working, and managing complex caseloads in the community setting.
- Experience of teaching, training and/or mentoring.
- Ability to use clinical data and digital systems to support decision‑making Confident using systems such as Rio/EMIS, producing caseload audits, tracking KPIs, analysing risk trends, and contributing to service improvement.
- Training in evidence‑based therapeutic interventions For example: CBT skills, DBT-informed practice, motivational interviewing, or family interventions to enhance care planning and recovery-focused work

- Strong partnership‑working skills with external agencies Able to liaise effectively with housing, social care, primary care, crisis services, voluntary sector providers, and inpatient teams to support holistic care.
