The post holder will be expected to liaise with medical, nursing, imaging and pharmacy staff to ensure all clinical measures are delivered ensuring appropriate care making use of a number of pathways and care bundles.
The post holder will support the increase in awareness and understanding across the Trust by sharing their specialist knowledge during day to day interaction with individuals and teams across the Trust as well as providing dedicated education sessions regarding AKI and fluid management.
Applicants are desired to have an existing knowledge of acute kidney injury and chronic kidney disease management.
We are looking for an enthusiastic, innovative candidate who would have the energy and drive to deliver a 1st class service.
The AKI Team provide a 7 day service from 8am – 8pm Monday to Friday, and 9-5pm during the weekend.
Main duties of the job
KEY DUTIES
To work autonomously, managing a caseload of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care.
Ensure key measures (AQ) are implemented for all patients with AKI that intend to improve patient outcomes such as length of stay, mortality and re-admissions. They are:
- Urine dipstick test carried out within 24 hours of first AKI alert
- Pharmacist Review of Medication
- Cessation of any medication that lowers blood pressure (ACE inhibitors and ARBs)
- Blood test (Serum Creatinine) to check kidney function within 24 hours of first AKI alert
- Ultrasound Scan within 24 hours of first AKI alert
- Discussion by specialists (Renal or Critical Care) within 12 hours of first AKI 3 alert
- Self-management information/patient information leaflet given to patient
Be responsible for ensuring medical staff complete the following (CQUIN) information in discharge summaries for patients with all levels of AKI:
- Stage of AKI (1,2,3)
- Evidence of Medicines Review
- Type of Blood Tests required on discharge for monitoring
- Frequency of blood tests required on discharge for monitoring

- Knowledge and understanding of the National agenda for the speciality, knowledge of quality standards, NICE Guidance, NHFD, CQC, NHSLA and Best practice

- Knowledge of current clinical and professional issues
Desirable
