Main duties of the job
Lead and manage Community Nursing Teams within a locality hub, with overall responsibility for assessing, managing, and evaluating evidence-based care across a range of community settings. Provide line management to District Nurses and Deputy Sisters/Deputy Charge Nurses, supporting day-to-day team operations. Promote effective multidisciplinary teamwork, ensuring appropriate delegation to enhance patient care and service delivery. Monitor and improve quality of care, identifying opportunities for service development and innovation. Hold overarching responsibility for team and individual performance within the hub, acting as a clinical role model and supporting the management of complex patient cases. Support the Community Services Matron in delivering high-quality, cost-effective care that meets commissioner requirements. Work collaboratively with key stakeholders to ensure integrated, seamless services, and act as a key point of contact for GPs and primary care teams at hub level.
