We are seeking an enthusiastic and motivated Project Manager to join our Integrated Care Operations Service (ICO)
This is an exciting opportunity to work at the heart of innovative integrated care programmes, supporting service delivery, operational improvement, data-driven decision-making, and partnership working across health, social care, and voluntary sector organisations.
Working closely with clinical and operational leaders, you will help shape and improve all ICO services including complex discharge, Inclusion Health patients, UCLH Hospital@Home as well as high intensity users, by providing project support and data function across the services.

Main duties of the job
This role centres on supporting the delivery and improvement of of integrated, person-centred services across health , social care, and voluntary sector partners, particularly for individuals with complex needs such as care needs, homelessness, mental health challenges, and substance dependency. The postholder works collaboratively with multidisciplinary teams to implement coordinated care pathways and promote inclusion health principles, while representing the service in partnership forums and ensuring strong relationships across acute, community, and external organisations.
From a leadership and operational perspective, the role involves managing and coordinating the administrative function, overseeing workloads, and providing line management, supervision, and development for staff. It requires facilitating meetings, supporting effective communication across teams, and ensuring a positive, values-driven team culture. The postholder also monitors operational performance, manages risks, supports service delivery, and ensures issues are escalated appropriately while maintaining high standards of governance and operational resilience.

The Integrated Care Operations (ICO) Department sits within the Emergency Services Division and comprises of the following services: • Camden Adult Pathway Partnership (CAPP) • Homeless Intermediate Care Team (HICT) • Rough Sleeper Outreach Nurse (part of the Homeless Health Outreach Project run in collaboration with North London Foundation Trust). • Integrated Care Operations administrative team • Inclusion Health Service • Integrated Discharge Service (including the Complex Safe Discharge team and Admission Avoidance) • Alcohol and Drug Liaison • Tobacco Dependency Service • UCLH@Home including the UCLH BloodTransfusion@Home The Department works in partnership with clinical colleagues across the Trust to collaborate with Integrated Care Boards, Local Authorities and community providers on the development of new models of care. We also work with our internal Emergency department and Patient Flow colleagues to reduce length of stay whilst supporting safe discharge. We are looking for a Project Manager who can work with the current Senior team to support and drive improvements and innovation to all the ICO services.

Essential
- Experience of operating through influence and networks rather than direct authority
- A proven record of success in managing operational change while also developing and maintaining high standards of quality
- Experience of working with partners and stakeholders in delivering sensitive changes to services
- Experience in operating in and managing diverse multidisciplinary team
- Evidence of managing competing priorities effectively within constrained timescales with limited resources
Desirable
- Experience of negotiations with commissioning bodies
Communication
Essential
- Excellent communication skills, including negotiation, with the proven ability to liaise effectively within a multidisciplinary team-based environment
Skills and Abilities
Essential
- Ability to build effective working relationships quickly and gain credibility when working with external partners and clinical colleagues
- Demonstrate a strong understanding of multidisciplinary issues and commitment to multidisciplinary working
- A clear understanding of the components of Clinical Governance and how these contribute to an integrated and quality driven service
- IT literate
- Project management, data and analytical skills
- Strong grasp of financial data, and ability to understand and interpret financial models
- Good organisational skills and abilities
- The ability and credibility to work co-operatively within senior management
- Ability to lead and develop a team effectively
Knowledge & Qualifications
Essential
- Significant knowledge of delivering effective integration services, data and project management
- Qualification in project management and/or Quality improvement
Desirable
- Professionally registered with extensive post registration experience in chosen professional field.
- Masters qualification or equivalent (or working towards same)
