Are you passionate about digital transformation and improving patient care? We’re looking for a motivated and detail-oriented Patient Pathway Co-Ordinator to support and improve the pathways within Specialist Medicine
This is a 12-month fixed-term opportunity to play a key role in transforming how we manage outpatient demand, helping to streamline pathways and reduce waiting times for patients. You'll work closely with operational, administrative, and clinical teams to ensure we continue to take positive steps in making a difference to our patients.

Main duties of the job
The management of PTL lists by specialty, liaising with consultants, nurses and administration team. Ensure effective maintenance of patient documentation and note holding systems.
Ensuring clinics schedules are up to date and sent across in timely manor
Deal with patient enquiries, either in person or by telephone forwarding queries to the appropriate person when necessary, taking into account Trust policy, with regards to equality and diversity, confidentiality etc.
Produce and maintain tracking spreadsheets and performance reports, escalating concerns where necessary.
Confident in the use of trust systems and MS software (Word, Excel, Powerpoint)
Plan daily duties and prioritse workload so this is completed in a timely manner and prioritised accordingly in keeping with KPI targets for the role. The postholder will work with some degree of autonomy and will be expected to problem solve for their patients within their remit.

- NVQ 4 or equivalent experience
- A Level in English and Maths
Knowledge and Experience

- Significant previous administrative experience within a healthcare setting and experience in using hospital systems
- Up to date knowledge and understanding of key access targets e.g. 18 weeks Referral to Treatment, 2 Week Wait
- Experience of working with IT office packages
