The Patient Follow-Up Coordinator is responsible for the day-to-day administration and coordination of Remote Monitoring and Supported Self-Management Pathways (SSMP) within the Cancer Support Team. The post holder will manage patient activity across remote monitoring systems, ensuring accurate data entry, monitoring pathway compliance, undertaking quality assurance checks, identifying and escalating data gaps, and tracking patients throughout their follow-up journey.
Working closely with Cancer Support Workers, Cancer Nursing teams, Prehabilitation services and other multidisciplinary colleagues, the post holder will support the delivery of safe, effective and patient-centred follow-up care for people living with and beyond cancer. The role requires strong organisational skills, attention to detail, and the ability to work independently to manage competing priorities while maintaining high standards of data quality and patient confidentiality.
In addition, the post holder will provide comprehensive administrative and project support to the Cancer Support Team, including coordinating meetings, maintaining records, supporting service improvement initiatives, and acting as a key point of contact for patients and healthcare professionals involved in remote monitoring pathways’

Knowledge
Essential
- Excellent customer care skills to communicate with patients and relatives, in person, and by telephone managing barriers of communication
- Ability to work as part of a team in a multidisciplinary environment
- Ability to communicate effectively with all multi-disciplinary professionals across all departments, and with GP practices and other Trusts
- Ability to act on own initiative within parameters set by service
- Ability to prioritise, organise own workload and to work to set deadlines
Desirable
- NVQ Level 3 or equivalent in Business/Admin
- GCSE (or equivalent) English and Maths grades A-C
Experience
Essential
- Admin and Clerical experience
Desirable
- Previous experience of working in a healthcare environment
