Pathway Coordinator ; General Surgery Department - Band 4

Full time 37.5 per week, Permanent

An exciting opportunity has arisen for a Pathway Coordinator to join the Pod 6 Administrative Team, General Surgery Department. The post holder will work as part of a multi-disciplinary Point of Delivery team (POD), and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed in a timely manner. The post holder will also act as the point of contact for patients regarding all administrative issues relating to the patients’ pathway of care.

Main duties of the job

  • Provide comprehensive administrative support for General Surgery, Colorectal, Vascular and Paediatric Surgery outpatient, diagnostic and treatment pathways, ensuring patients receive timely and effective care.
  • Coordinate patient appointments, diagnostic investigations, procedures and follow-up bookings, ensuring pathways are managed efficiently and in line with clinical priorities.
  • Manage referrals, waiting lists and pathway activity in accordance with Trust policies, national waiting time standards and Referral to Treatment (RTT) requirements.
  • Monitor patient pathways proactively, identifying delays, incomplete actions or potential breaches, and take appropriate action to support timely progression through care.
  • Maintain accurate and up-to-date patient records on Trust information systems, ensuring data quality and supporting pathway validation processes.
  • Act as a key point of contact for patients, carers, clinicians and multidisciplinary teams, providing a professional, responsive and patient-centred service.
  • Prepare clinic documentation, process clinical correspondence and support the effective administration of outpatient clinics across General Surgery (Upper GI), Colorectal Surgery (Lower GI), Vascular Surgery and Paediatric Surgery services.
  • Liaise with diagnostic services, theatres, specialist teams and external organisations to ensure a coordinated approach to patient care and treatment planning.
  • Support waiting list management activities, including validation, pathway tracking and patient communication, to ensure compliance with RTT and Trust performance standards.
  • Attend and contribute to weekly Patient Tracking List (PTL) meetings, providing accurate pathway information and highlighting any risks, delays or concerns.
  • Produce, maintain and update pathway information, ensuring accurate data entry, reporting and information management to support service performance and operational targets.
  • Communicate effectively with patients, clinicians and colleagues across the wider organisation, resolving queries and facilitating the smooth progression of patient pathways.
  • Work collaboratively with clinical and administrative teams to support continuous service improvement and deliver a high-quality patient experience.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

  • Significant administrative experience in an NHS, healthcare, or similar environment.

  • Knowledge of NHS administrative processes and patient booking systems.

  • Experience of using Cerner, or other NHS clinical systems.
  • Experience producing performance reports and analysing data.

  • Competent in Microsoft Office applications, particularly Outlook, Word and Excel.

Education and Qualification

Essential

  • GCSE or equivalent, Grade C or higher in English and Maths

  • Significant administrative experience in an NHS, healthcare, or similar environment
