The Advanced Nurse Practitioner (ANP) provides autonomous assessment, diagnosis and management of patients presenting with a wide range of urgent and primary care conditions within the Primary Urgent Care Centre (PUCC). The role involves taking comprehensive patient histories, undertaking clinical examinations, analysing and interpreting clinical information to support decision-making, and developing appropriate management plans. The ANP works closely with colleagues across the Emergency Department and wider multidisciplinary team to ensure safe, effective and patient-centred care, while also contributing to clinical leadership, education, service development, audit and quality improvement activities.

Main duties of the job

- Assess, diagnose and manage patients presenting with a wide range of urgent and primary care conditions autonomously.

- Take comprehensive patient histories, undertake clinical examinations and interpret clinical information to support diagnosis and management decisions.

- Develop, implement and evaluate treatment plans, including discharge, referral or escalation of care as appropriate.

- Work closely with colleagues across the Emergency Department and multidisciplinary teams to deliver safe, effective and patient-centred care.

- Perform advanced clinical procedures and interventions within scope of practice.

- Promote health education, wellbeing and preventative care for patients.

- Provide clinical leadership, supervision and support to junior staff, students and trainees.

- Contribute to service development, clinical governance and quality improvement initiatives.

- Participate in teaching, mentorship, audit, research and the development of clinical guidelines and pathways.

- Maintain accurate clinical records and ensure care is delivered in line with Trust policies, professional standards and evidence-based practice.

Detailed job description and main responsibilities

Clinical Responsibilities

• Undertake comprehensive patient assessments, including history taking and physical examination.
• Analyse and interpret clinical information, such as x-rays and blood test, to support diagnosis and clinical decision-making.
• Develop and implement appropriate management plans, including treatment, discharge, referral and escalation of care where required.
• Independently manage patients with undifferentiated and undiagnosed presentations within scope of practice.
• Perform advanced clinical skills and procedures relevant to the service.
• Identify and respond appropriately to medical emergencies and deteriorating patients.
• Provide health promotion, education and advice to patients and carers.

Communication and Team Working

• Communicate effectively with patients, relatives, carers and members of the multidisciplinary team.
• Work collaboratively with Emergency Department clinicians and specialty teams to ensure timely and appropriate patient care.
• Provide specialist advice and support to colleagues regarding patient management.

Leadership and Management

• Support the day-to-day operational management of the service.
• Contribute to workforce planning, rota management and maintaining safe staffing levels.
• Monitor standards of care and contribute to clinical governance, risk management and service improvement initiatives. [
• Contribute to the development and review of clinical pathways, protocols and guidelines.

Education and Development

• Act as a mentor, clinical educator and role model for students and junior staff.
• Participate in teaching, training and supervision activities.
• Maintain professional competence through continuing professional development and reflective practice.

Quality Improvement and Governance

• Maintain accurate and comprehensive clinical records.
• Participate in audit, research and quality improvement projects to enhance patient care and service delivery.
• Ensure practice complies with Trust policies, professional standards and evidence-based guidelines.
