This is a part time post working 24 hours per week. This post is only open to current employees of the Trust.
You must have read and understood the terms of the Trust's secondment policy and gained permission from your Line Manger to undertake the secondment before applying.

The teams operate 7 days a week between the hours of 07.30- 18.30 and shift times are variable within these hours, i.e. 07.30 15.30, 08:30 16:40, 10:30 18.30, You will be supported to work flexibly within these hours.

You will be involved in team meetings, clinical audit, staff management and team development. Working with the team leaders, your input is a valuable part of the teams work.
Main duties of the job
Your typical working day will include coordinating and supporting the Wyre Forest Neighbourhood teams caseloads, you will support triage and allocate planned and unplanned visit requests for nursing interventions. You will clinically supervise/ support the Wyre Forest integrated team and support the clinical overview of the integrated work across the Neighbourhood teams.
This may involve urgent response assessment as part of the teams admission avoidance remit, supporting complex hospital discharges including complex care needs and planning domiciliary interventions to facilitate independent living. You will work alongside nursing, physiotherapy and OT colleagues in the same team. The role includes clinically supporting the workforce, mentoring new and existing staff, supporting with complex clinical issues and supporting operational delivery of the service. Also to include mentoring students and as we have close links with Worcester and Birmingham universities in support of these student placements.

Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions to patients either in their own homes or other community settings. Provide case management for a designated caseload of patients who have immediate and / or complex care needs.

Work holistically with all team members, carrying out joint visits and actively participating in the integration of services, increasing the knowledge of all team members through case discussion and joint working.

Be responsible for the timely assessment and treatment of patients who have urgent and/ or complex primary care needs
Co-ordinate appropriate interdisciplinary, patient centred care that promotes independence for patients on their caseload
Provide health care advice, clinical supervision and education to team members, statutory, voluntary and independent sector providers
Be responsible for health promotion, teaching and training of support workers, preregistration students, patients, relatives and carers

- Evidence of relevant post graduate courses / Continual Professional Development maintained in portfolio
