An Exciting New Opportunity

An exciting opportunity has arisen to join the Mid Furness and Millom Integrated Care Community (ICC) Team as a Band 6 Case Manager.

This is an excellent development opportunity for an experienced Band 5 Community or District  Nurse, looking to progress into a new and rewarding clinical role. It is also an ideal opportunity for an existing Band 6 practitioner wishing to broaden their experience within the Integrated Care Community model and contribute to the development of innovative, person-centred care across Mid Furness and Millom.

As a Case Manager, you will play a key role in supporting individuals with complex health and social care needs, helping them to maintain their independence, improve their wellbeing and achieve the outcomes that matter most to them.

You will provide clinical leadership and support to a team of Care Co-ordinators, Care Navigators and Administrative staff, working closely with the Clinical Team Lead, ICC Clinical Lead, ICC Development Lead and colleagues across VCFSE sectors.

Main duties of the job

The successful candidate will provide holistic, evidence-based care to individuals with complex needs, undertaking comprehensive assessments and reviews within patients' homes and community settings.

The role will involve:

  • Managing a complex caseload of patients with long-term conditions, frailty and multiple health and social care needs.
  • Co-ordinating care across multidisciplinary teams to ensure seamless and person-centred service delivery.
  • Supporting patients to remain independent at home through proactive care planning and intervention.
  • Developing, implementing and evaluating personalised care plans in partnership with patients, carers and professionals.
  • Providing clinical advice, guidance and signposting to appropriate services and support networks.
  • Leading holistic care planning and promoting shared decision-making.
  • Supporting proactive case-finding and population health management initiatives.
  • Monitoring patient outcomes and contributing to continuous quality improvement.
  • Acting as a role model and mentor for colleagues, supporting learning and professional development.
  • Contributing to service development and the ongoing evolution of the ICC model across Mid Furness and Millom.
