The Lead Nurse will provide visible clinical and professional leadership of the integrated discharge team, the Lead Nurse post is a senior leadership role focused on patient flow, complex discharges, and delayed discharge reduction. The post holder will be responsible for leading and managing the Hillingdon Hospital Integrated Discharge Team, delivering a robust integrated discharge model that ensures effective, person-centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols.
