Inpatient Heart Failure Specialist Nurse

Job summary

Are you an experienced Specialist Nurse and interested in working in the Heart Failure Team, we have 2 fixed term posts within the team and would encourage you to apply if you would like to work in this team in the following way.

To join the existing highly developed integrated, nurse led heart failure service.

To ensure that the service standards meet the requirements of the National Institute of Clinical Excellence guidelines CG187 and NG106.To provide evidence-based specialist nursing assessment, advice to patients with a confirmed diagnosis of Heart failure with either a preserved ejection fraction or a reduced ejection fraction and support to nurses in the acute or community setting.

To facilitate Primary Health Care Teams and local hospice teams to develop effective and appropriate heart failure pathways.

To work closely with St. Marys colleagues, CCG colleagues, General Practices, local hospice and Tertiary centres in providing and developing a service that will meet and maintain standards across primary/secondary care pathway.

Main duties of the job

The post holder will manage a caseload of patients, arrange follow up appointments.

To develop and use individual clinical management plans required for independent and supplementary nurse prescribing.

Discharge patients when medication optimisation is complete.

Uses a structured approach for obtaining the history, exploring all patient problems and complaints and adapting history taking as appropriate to method of review, surroundings and patient needs.

Discusses the findings of the assessment with the patient and carer in an appropriate manner which they understand and informs the patient of the action plan and any investigations ordered based on the findings.

Provides the patient with primary and supporting information relating to heart failure in a manner and language that is understood by patient and carer. Supporting information includes literature, websites and videos.

Co-ordinate and plan liaison and case conferences with the multi-disciplinary team to ensure effective case management of patients with heart failure.

Demonstrates knowledge of the evidence base that underpins the treatment of patients with heart failure,

Recognises the interaction of co-morbidities such as diabetes with heart failure and take into account when making a differential diagnosis; how therapies used to treat co-morbidities impact on cardiovascular health and vice versa, the role of co-morbidities in the development of heart failure and the impact of frailty.
