
Job summary
Nottingham CityCare Partnership is seeking an enthusiastic and motivated Health Inequalities and Community Engagement Lead to help shape how we understand, engage with and respond to the needs of the diverse communities we serve.
Working within our Nursing and Quality Directorate, you will play a key role in helping CityCare deliver better outcomes for patients, families, communities and staff by ensuring that equity, inclusion and engagement are embedded throughout our organisation.
This role is permanent, part time, working 2 days per week.
This is an exciting opportunity to lead the development, coordination and delivery of CityCare's health inequalities agenda, ensuring that equity, inclusion and meaningful community engagement are embedded across all areas of service delivery.

Main duties of the job
You will lead the implementation and development of our Health Inequalities Framework, ensuring that national priorities such as Core20PLUS5 are translated into meaningful local action. You will oversee a range of projects and initiatives aimed at improving equity of access, experience and outcomes across our services.
You will also play an important role in providing assurance and governance support through attendance at Health Inequalities, Experience and Engagement and other relevant forums, producing high-quality reports and insight that inform decision-making at all levels of the organisation.

Job Purpose
The Health Inequalities Lead will provide leadership and coordination of CityCare's health inequalities agenda and community engagement approach, supporting the organisation to reduce unwarranted variation in access, experience and outcomes.
The post holder will:
- Lead the oversight, coordination and delivery of the organisations Health Inequalities Framework and associated workstreams including oversight of the organisations contribution to Core20PLUS5 priorities at place and system level.
- Drive forward programmes and projects that address health inequalities and improve equity across services.
- Ensure that meaningful engagement with underserved and seldom-heard communities informs service design, improvement and evaluation.
- Support governance and assurance through attendance at relevant meetings and the production of high-quality reports.
- Work closely with operational, clinical and corporate teams to embed health inequalities and engagement principles within service delivery.

- Responsible for direct budget management and for ensuring delivery of programmes within agreed resources and contributing to business cases where required.

Key Responsibilities
Leadership and Strategy
- Lead the implementation, delivery and continuous development of the Health Inequalities Framework.
- Translate national and system priorities (e.g. Core20PLUS5) into local delivery plans.
- Oversee and monitor CityCare's contribution to Core20PLUS5 priorities, ensuring alignment with Integrated Care System (ICS) objectives and demonstrating measurable impact on identified population groups.
- Provide expert advice to services on reducing inequalities in access, experience and outcomes.
- Support the integration of health inequalities within quality improvement, governance and service redesign.
- Build relationships with community organisations, voluntary sector partners and stakeholders.
- Facilitate co-production approaches to service design, improvement and evaluation.
- Programme and Project Management
- Oversee a portfolio of health inequalities projects, ensuring clear objectives, milestones and measurable outcomes.
- Monitor and report on progress against key priorities and organisational objectives.
- Identify risks, gaps and opportunities, escalating and mitigating appropriately.
- Data, Insight and Reporting
- Work with Business Intelligence and operational teams to analyse inequalities data and identify priority areas.
- Produce high-quality reports for governance groups, including Health Inequalities, Participation, Engagement and Partnership Group and Quality Committee.
- Provide assurance on progress, impact and outcomes of health inequalities initiatives.
- Support development of dashboards and reporting mechanisms to track inequalities.
- Contribute to the development and monitoring of Core20PLUS5 metrics and reporting, ensuring robust tracking of outcomes and progress across priority cohorts.
Governance and Meetings
- Attend and contribute to CityCare committee and subgroup meetings.
- Lead preparation of papers, reports and presentations for internal governance forums.
- Ensure actions and outputs from meetings are tracked and progressed.
Partnership Working
- Work collaboratively with Integrated Care System (ICS), Local Authority and voluntary sector partners.
- Represent CityCare at relevant system-wide forums relating to health inequalities.
- Promote alignment between organisational and system priorities.
- Embedding Culture and Practice
- Promote a culture of inclusion, equity and continuous learning.
- Support staff to understand their role in addressing health inequalities.
- Develop and deliver training, awareness and guidance for teams.
- Support and contribute to organisational forums focused on experience and engagement (e.g. SHARE), ensuring:
- Representation of underserved and seldom-heard communities
- Integration of health inequalities insight into discussions and decision-making
- Evidence that feedback reflects diverse population groups
- Training and Learning Support
- Develop, deliver and evaluate training, guidance and awareness resources to build organisational understanding of health inequalities, Core20PLUS5 priorities, inclusive engagement and equity-focused improvement.
- Support services to build confidence and capability in recognising and responding to health inequalities, including use of data, patient/community feedback, inclusive engagement approaches and targeted improvement actions.
- Work with managers, clinical leads and teams to translate learning and insight into practical changes that improve access, experience and outcomes for underserved and seldom-heard communities.
- Identify learning needs across services and support the development of tailored resources, briefings and tools to embed health inequalities considerations into everyday practice.
- Share learning, good practice and examples of impact across the organisation to support continuous improvement and strengthen a culture of inclusion, equity and learning.

Experience
Essential
- Experience of leading or coordinating health inequalities, population health, inclusion & engagement work.
- Experience of delivering projects or programmes that improve access, experience or outcomes for patients, communities or population groups.
- Experience of community engagement, involvement or co-production
Desirable
- Experience within NHS or community health setting
- Experience of working with VCSE sector
- Experience of developing or delivering training, guidance or awareness sessions for staff.
Qualifications
Essential
- Educated to degree level or equivalent experience
- Evidence of continuous professional development
Desirable
- Qualification or training in public health, community engagement, community development, quality improvement, leadership or project/programme management.
Knowledge and skills
Essential
- Strong knowledge and understanding of health inequalities, population health and the wider determinants of health.
- Strong report writing skills, including the ability to produce clear, evidence-based reports for governance, assurance and senior decision making.
- Effective project and programme management skills, including planning, delivery, risk management, monitoring and evaluation.
- Skills in community engagement, involvement and co-production, including working with underserved and seldom-heard groups.
- Ability to engage, influence and build relationships across diverse groups
