Main duties of the job

As a Care Coordinator, you'll be a key point of contact for patients, carers, and professionals, ensuring joined-up support across health, social care, and voluntary services. You'll manage a caseload, coordinate care, and help patients access the right services to maintain their independence and wellbeing.

In this rewarding role, you will:

• Support housebound patients living with long-term conditions.
• Help reduce avoidable hospital attendances and admissions.
• Respond to referrals and coordinate access to services.
• Provide advice and support to patients, carers, and colleagues.
• Develop personalised plans that promote independence and prevent care crises.
• Empower patients to proactively manage their health and wellbeing.
