Job summary

The Dynamic Support Pathway (DSP) coordinates support for people with a confirmed diagnosis of Learning Disability, Autism, or both who are experiencing a decline in their mental health whilst living in the community. The teams purpose is to bring involved professionals together, and agree additional support to prevent deterioration, crisis, or hospital admission. The DSP also supports people stepping down from inpatient units, co-ordinating discharge planning and post discharge support.

The pathway applies to both adults and children/young people (CYP), using the same referral process.

The service operates Monday to Friday 9-5.

An exciting opportunity has arisen for a Band 7 Team Lead to provide leadership and management to the DSP Team ensuring that the existing standard operational procedures and relevant local and national policies are implemented to a high standard. The post holder will also hold some operational responsibilities in relation to LDA Discharge Hub Case Managers.

The successful candidate will have experience of working with people with learning disabilities and/or autism and recognise the significance of early interagency collaborative working, and intensive support.

Main duties of the job

  • Oversight of all DSP referrals, triaging content and ensuring multi agency meetings occur within required timeframes. Chair MAM and LEAP meetings. Provide clinical and operational leadership within the DSP team, ensuring governance, data review and high quality practice. Work collaboratively across the LLR system, leading discussions in MAMs to identify risks and coordinate multi agency intervention plans. Support wider system partners by identifying admission themes and contributing to service development.
  • Hold responsibility for care quality, providing professional nursing leadership across key areas: patient safety, effectiveness of interventions, patient experience and care environments. Ensure delivery of a clinically effective service aligned with Trust policies and national/local plans. Coordinate services to promote health, self management and effective inter agency liaison.
  • Carry 24 hour accountability for staff, budgets, quality and standards, including supervision, development, competency planning, audit and policy compliance. Ensure personalised care is delivered in line with Trust standards. Provide strong leadership to sustain a positive culture, ensuring staff are trained, supervised and competent. Act as an influential senior nurse, offering expert advice internally and externally, promoting the speciality and fostering transparent, collaborative MDT relationships.
