If you are, an exciting opportunity has arisen for an ambitious and enthusiastic Clinical Lead Nurse to join our District Nursing Service.
We are looking for District Nurse who holds a Community Specialist Practitioner Qualification with significant Community Nursing experience to lead a Primary Care Network (PCN) aligned Locality team. The post holder will need to demonstrate a comprehensive specialist knowledge base and promote excellence in nursing practice in a dynamic healthcare environment.
They will be responsible for the development and delivery of a 7 day consistently high quality, safe and sustainable, cost efficient service operating 08:00 till 18.00
Main duties of the job
You will provide clinical leadership and line management support to a Community Nursing team, working closely with operational leads to deliver high-quality patient care.
The role includes assessing new and complex patients in their own homes, planning, implementing, and evaluating nursing interventions and care plans. You will support delivery of the Enhanced Health in Care Homes Framework in line with local service objectives.
You will lead continuous quality improvement initiatives and contribute to effective clinical governance and caseload management within the team. In addition, you will support patients and carers to achieve maximum independence through evidence-based practice while adhering to national and Trust policies and guidelines.
