We are excited to announce that we have a vacancy in the following post:
Deputy Adult Community Services Lead – Stort Valley & Villages and Royston Integrated Care Teams – 30 hours per week.
The post holder will be expected to use a range of reports and approaches to identify key areas for improvement within the team and to work closely with community matrons, the Team Leads and the Adult Community Services Lead to implement change successfully.
The role of the Deputy Adult Community Services Lead is a pivotal one in the team, to be able to deliver the Integrated Care Team in line with the service specification to deliver the key performance and quality indicators.
Whilst the role is predominantly non-clinical the post holder is expected to maintain their clinical competence and act as a clinical role model.
We are looking for someone who is passionate about contributing to the success of the service.
In addition to the basic salary, this post attracts a High-Cost Area Supplement.
A full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010). Travel to additional sites will be part of the job role.

Main duties of the job
Responsible for the day to day management of both the team and caseload, ensuring adequate staffing levels are maintained and a high standard of care is achieved meeting patient expectations.
Ensure team/staff provide safe clinical care in patients’ homes through agreed pathways concentrating on the demanding/complex cases that may require advanced assessment and management skills.
Provide on-going support to those patients, families and carers with complex needs, where there may be emotional, physical and life threatening conditions. Provide support to other team members in relation to this work.
Ensure the maintenance of adequate staffing levels, utilising e-rostering software as appropriate, meeting the required timelines for roster sign off. Risk report and escalate to line manager where required resource cannot be achieved.
