Job summary

Are you energetic, motivated and want to lead a busy MDT. We seek a Team Lead to join the North Home Treatment Team which provides a responsive assessment and home treatment service to those experiencing mental health crisis.

HT provides intensive care to patients and are looking for individuals who have leadership skills that encompass urgent mental health care and can lead and encourage our clinicians to provide the best care possible to our patients

HT is integral in providing a specialist service, comprehensive assessment, treatment and care. We work collaboratively with service users, carers and professionals offering interventions on an alternative to hospital admission basis.

We support early discharge from inpatient wards through intensive, time limited intervention in the community. HT also supports primary care through screening/assessment of urgent GP referrals. Positive risk management is vital in this role.

Main duties of the job

To clinically and operationally lead a large, skilled and experienced clinical team within the Urgent Care Pathway.

To manage a defined caseload of patients experiencing an acute mental health episode.

To support the team to identify early facilitated discharges from inpatient wards and work alongside the NMHT to prevention admissions, caring for people in their homes, following the least restrictive approach.

To assist in identifying, developing and providing a multidisciplinary approach within the service in accordance with the needs of service users and the requirements of the service.

A strong focus on operational responsibilities including (but not limited to) workforce management, governance adherence, supervision, appraisals, recruitment and retention

Supporting the education and development of our clinicians to ensure we continue to provide the most effective and evidence-based care as possible, in line with patients needs.

To contribute to the planning and implementation of induction, training and education of students and new staff.

You will actively participate in the development of the service and participate in the overall development of the service ensuring the principles of Clinical Governance underpin all developments.

To work closely with the Service Lead and other Team Leads within Home Treatment to ensure we are supporting our staff, patients, carers and wider MDT in the delivery of safe, effective and evidence based care.

To have a strong focus on the operational management of a busy Urgent Care Team.

To improve the lives of people with mental ill health and /or learning disabilities by minimising the impact of their condition through the delivery of excellent services to promote recovery and wellbeing.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and line management supervision to junior clinicians and students as appropriate.

Undertakes risk assessments/risk management in accordance with the Trusts Clinical Assessment and Management of Risk to Self and Others for Patients in Mental Health Services Policy, devising and implementing actions and intervention plans which take the risk formulation fully into account.

The ability to develop, implement and evaluate intervention plans as part of the multi-disciplinary team, with the involvement of the patient and where appropriate their carer.

Knowledge

Essential

  • Knowledge of relapse prevention.
  • Leadership, Management and change management.
  • Broad knowledge of risk assessments.

Desirable

  • Knowledge of issues involved in the treatment of mental illness and relapse prevention.

Experience

Essential

  • Three years post-registration experience 2 years at a senior level in Adult Acute mental health.
  • Experience of working with severe and enduring mental illness.
  • Management and leadership in a multidisciplinary environment.
  • Budget management.
  • IT literate.
  • CRT experience.

Desirable

  • Psychosocial interventions.
  • Use of clinical assessments.
  • First aid training.
  • Leadership.
  • CRHT experience.

Qualifications

Essential

  • A current professional qualification (RMN, RMNH).
  • Demonstrate management in a multi-disciplinary team setting.
  • Evidence of post qualification training / studies.
  • Teaching and assessing qualification.
  • Supervision Training.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Desirable

  • Qualification in Health Care at a degree level or equivalent.
