An exciting opportunity has arisen to join Queens Park and Paddington Community Mental Health Hub (CMHH) as a Clinical Team Lead.
We are looking for an experienced Mental Health Nurse to join our dynamic team. The post holder will work with adults in the community with mental health needs and support the team with the day to day running of a busy mental health service.
The role is suitable for a qualified clinician with experience working in the community and for someone seeking to develop their leadership and management skills.
Main duties of the job
The post holder will work as a Team Leader in the Hub with duties including:
- To coordinate referrals and transfers of care
- Be responsible for the supervision of staff & their case load.
- Manage a small caseload
- Support coordination of duty
- Develop and maintain close professional relationships with different teams both within Westminster and external partners
- Supports clinical governance processes, including reviewing Datix incidents and contributing to serious incident investigations under PSIRF.
- Ensure staff practice in a safe, appropriate and professional way with service users and their carers.
- Attend professional practice forums
- Chair clinical and team meetings as appropriate
- Represent the CMHH at various meetings
- Provide senior support to the Team Manager
- To assist the team to be compliant with KPI and monitor electronic records
- Supports staff appraisal, supervision, induction, training and ongoing team development.
Detailed job description and main responsibilities
We’re looking for a confident, proactive Clinical Team Lead who thrives on both hands-on practice and broader service development. You’ll carry a small but complex caseload while providing professional and clinical leadership to your multi-disciplinary team, mentoring staff, coordinating inductions and training, and fostering a supportive, learning-focused culture.
You will drive the design and implementation of community pathways that emphasise recovery, skill-building and prevention, working collaboratively with the QPP senior leaders to translate strategic priorities into everyday practice.
Collaboration is at the heart of this role: you’ll partner with inpatient wards, Home Treatment Teams, primary care, social services and voluntary sector organisations to ensure seamless, person-centred transitions across the care pathway.
In addition, you’ll champion quality improvement through audit, data-driven reviews and innovation projects, all underpinned by a commitment to compassionate, inclusive care.
