An exciting 12-month, Fixed Term or Secondment (maternity leave cover) opportunity for a highly motivated, innovative & confident Clinical Specialist Nurse (CSN) to work with the Memory Assessment Service across Scarborough Whitby & Ryedale (SWR) locality.

This is an exciting leadership post, specifically commissioned to work collaboratively with Voluntary Care (VCSE) colleagues, Primary Care and the Memory Service to promote the redesign of the system-wide memory service pathway. This role will pave the way for future systemic ways of working that will streamline diagnosis and support for people waiting for memory assessments, increase the number of memory assessments and diagnosis delivered safely within Primary Care, improve the quality and number of referrals to the specialist Memory Services, and enhance the overall experience for patients, their carers and families.

You would be a valued member of the Memory Service Leadership Team, prioritising the development of relationships with Primary Care, VCSE and other place-based organisations to establish effective and simple processes that will develop clinical skills and practice, enabling memory assessment and diagnosis within Primary Care.

Applicants should be a Registered MH Nurse with excellent knowledge of memory assessment and dementia diagnosis across the community system, including primary care, specialist services, local authority & VCSE.

Main duties of the job

The successful post holder will provide specialist advice, guidance, training, therapeutic support, and consultation to Primary Care and the VCSE supported by the Memory Assessment Service Leadership Team, including a Psychiatrist, Psychologist, CSN and Team Manager.

You will continue to develop and test the necessary relationships, systems and processes to provide expert advice and consultation to Primary Care, enabling the safe, timely and proportionate diagnosis of Dementia for appropriate patients within that setting. You will develop means of supporting the evaluation of the new partnership approach in order to inform the wider redesign work, sharing good practice and learning across Memory Services in North Yorkshire and York.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

This post is to cover maternity leave for 12 months and is open for Secondment or Fixed Term applicants - If  you are applying as a Secondment opportunity, you will need the prior agreement of your current Line Manager to be considered for shortlisting and interview.

You will work collaboratively, assuming an active and proactive leadership role in the establishment of partnerships and networks with key stakeholders across the wider mental health care system, creating a culture of ‘partnership’ between people and services, promoting clear communication and improved access to specialist advice, knowledge and skills.

Due to the need to be able to support Primary Care teams with diagnosis and treatment, it is essential that you are a Non-Medical Prescriber or can demonstrate that you will be able to become a Non-Medical Prescriber within an agreed timescale.

You will contribute to increasing community awareness of the presentation, prevalence and prevention of Dementia in local community populations, using this insight and intelligence to inform future redesign improvement activity and decisions about investment.

Knowledge

Essential

  • Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally
  • Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act
  • Detailed understanding of Safeguarding and its application in practice.
  • Clinical Risk Assessment and Management and its application in practice.
  • Clinical Governance and its application in practice.
  • Working knowledge of clinical medicines management
  • Understanding of psychological models of care and treatment (depending on specialist area)

Experience

Essential

  • Significant experience working with patients with complex needs in the specialist field.
  • Significant experience working with patients who display risk behaviours

Qualifications

Essential

  • Current Professional Registration with Nursing and Midwifery Council - RMN / RNLD
  • Recognised Practice Assessor/Practice Supervisor or currently in progress of achieving.
  • Trained in Clinical Supervision and providing Clinical Supervision or in the process of achieving
  • Professional clinical knowledge and experience acquired through degree (or equivalent post registration study), supplemented by specialist training, experience, short courses at level 7, for example Postgraduate Certificate/Diploma/Masters Degree, Intermediate Systemic Family Therapy/Postgraduate diploma in CBT/British Isles Dialectical Behaviour Therapy /NMP (Independent Prescriber), Psychosocial Interventions,
  • Within the first year of being in post hold a recognised qualification in a governed therapy/advanced clinical qualification e.g Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy /NMP (Independent Prescriber)
  • And have Equivalent clinical experience to that of a Post Grad Diploma/master’s level such as working as a band 6 (or above) in relevant clinical area with significant experience (see below under experience), together with evidence of relevant CPD
