Thank you for your interest in this Band 6 Mental Health Practitioner position within our Ealing Community Rehabilitation Service (CRS). This is a full-time and permanent position and attracts 15% Outer London weighting. We welcome applications from both Registered Mental Health Nurses (registered with the NMC) and qualified Social Workers (registered with Social Work England).
West London NHS Trust’s Community Rehabilitation Services have been operational for approximately 5 years and have recently gone through a period of review. We are in the process of implementing the findings of that review, which include a shift from the Care Coordinator role towards a named-worker model that allows for more specific professional and clinical input to occur. The emphasis of these changes is to ensure that the service meets and exceeds the current quality standards for community rehabilitation for psychosis, and aims to provide the highest standards of clinical care possible
Main duties of the job
Provide comprehensive biopsychosocial assessments, reviews and interventions to service users.
Be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users.
To contribute to working in partnership with service users in the designated care group with complex health presentations, developing their care and support plans to enable them to lead fulfilling lives.
Work at all times to promote the safety and the wellbeing of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students. To contribute to work in partnership with service users in the designated care group with complex health presentations, developing their care and support plans to enable them to lead fulfilling lives.
Provide expertise to the health and social care agenda within community services, ensuring timely and proactive care planning is offered to users and their families to the best possible standard in keeping with the Care Act 2014.

- Knowledge of counselling skills/cognitive behavioural skills, psychosocial interventions
- Knowledge of recovery approaches in mental health
- Advanced knowledge of care planning processes
- Specialist knowledge of mental health conditions as well as their relationship to physical health
- Experience of social work assessment and interventions
Experience
Essential
- Demonstrable experience of post qualifying work as a Social Worker/Mental Health Nurse in a mental health setting
- Experience of working with patients and their carers with complex health and social care difficulties.
- Understanding and experience of operating with a comprehensive assessment framework
Desirable
- Experience of working with people within a social care model of mental health
- Evidence of experience in a Band 6 position or equivalent
- Experience of conducting a range of clinical assessments including risk assessments.
Qualifications
Essential
- Professional qualification in Social Work, with current Social Work England registration or professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
- Attained at least Level 2 Maths and English (equivalent to GCSE grade 9-4 (A*-C))
- Educated to degree or Diploma level.
Desirable
- Evidence of recent and on-going relevant further professional development.
