JOB DESCRIPTION

POST: Adult Nurse Public Health

For those experiencing Homelessness

LOCATION: Belfast Inclusion Health Service & Homeless Facilities

BAND: Band 7

REPORTS TO: Lead Nurse

RESPONSIBLE TO: Service Manager

Job Summary / Main Purpose

The Belfast Inclusion Health Service BHSCT is a nurse led multidisciplinary team initiative and is operational among an identified population who experience homelessness.

.

This population suffer complex and diverse health and social care needs including physical health, mental health, substance misuse issues and adverse Childhood/adulthood experiences.

Many service users experience access barriers in seeking medical help, advice and support.

The service aims to transform how services are delivered to those experiencing homelessness including those who are street sleeping within the Belfast Area.

The service operates an "outreach" delivery model bringing services within hostel facilities, shelters and for those with no fixed abode on the street.

The Belfast Inclusion Health Service is looking for a highly motivated individual to join their multidisciplinary team. Interest is welcome from those who are registered on Part 1 of NMC Register (Adult) and have experience in working with vulnerable groups and those experiencing homelessness.

Main Duties / Responsibilities

Part of this role would be to promote the ethos of Inclusion Health Care for this population including: -

Working with individuals in a trauma informed way to gain insight into lifestyle behaviors and life choices to enable meaningful interventions.

Challenging access barriers and advocate to ensure equity of health and social care services.

Promote a culture of empathy and advocacy among staff and address issues of poor performance or stigmatizing behavior appropriately.

Assist the health care coordinator to provide leadership that facilitates professional decision making and effective team working.

Develop pathways and communication across a range of health care services that are involved with this population

Enhancing liaison and partnership working between hospital and community care for improved health outcomes

Developing dedicated pathways with Statutory, Voluntary and the Community sector agencies in delivering together to address homelessness and health collectively

Developing joint protocols with partnership agencies that allow meaningful changes for this population

Supporting colleagues to improve health outcomes for those experiencing homelessness.

Work in a multidisciplinary way with other health teams such as: - drug outreach, substitute prescribing, custody, hepatology, genitourinary medicine and other disciplines within the team.

Assist in taking forward the findings of service developments to improve future service delivery through quality improvement initiatives.

Benchmark and auditing Specific Strategic directives.

Maintain own personal and professional development in accordance with NMC code of professional conduct and within the scope of professional practice.

Communicate effectively with all members of the multi-professional team to ensure that service user needs are met.

Ensuring staff are aware of and work in line with Trust policies.

Ensuring vulnerable adults and safeguarding is adhered to when necessary.

Ensuring that the requirements of this specialist role are implemented and managed to define standards and operating procedures.

Being influential within the organization in matters relating to their area, providing detailed advice and guidance to other specialties as required

Being active in the orientation and induction of all new staff.

Leading activities that contribute to the continuous development of education and efficient team processes.

Ensuring there are clear protocols for the management of poor practice and/or performance

Clinical

For each of the following, the post holder will;

Act as a role model and enable effective communication with those experiencing homelessness.

Autonomously assess, diagnose, treat or refer within your scope of practice.

Demonstrate clinical expertise by role modelling person-centered practice to others.

Deliver skilled care and treatment for a range of illnesses and injuries.

Actively participate in all clinical aspects of the service including; - screening and Immunisation Programs. e.g. Blood Borne Virus Screening, sexual health screening

The post holder will ensure follow-up and treatment pathways are adhered to.

Ensure skills and knowledge in interpreting test results, follow-up and referral to secondary care services as appropriate.

Make recommendations regarding care regimes or referrals to external health providers.

Communicate effectively to service users when there is a clear barrier to understanding treatments and outcomes.

Ensure that all treatment protocols are systematically and continually evaluated and if necessary, develop new protocols as appropriate.

Adopt a questioning approach to existing practices and constantly seek ways of improving care delivery based on available evidence. Ensure systems and processes are in place to support and lead nursing staff in providing patient-centered care to agreed standards.

Act as an advocate when relevant and to be able to educate on all the treatment options to enable patients to make informed choices.

Ensure adherence to the DHSS and NMC guidelines for the safe handling, administration, and storage of medication.

Demonstrate professional standards of leadership, supervision and clinical management.

Maintain accurate, timely clinical records and have knowledge of Information Technology systems used within the BHSCT.

Maintain the clinical facilities within the inclusion premises complying with professional standards and guidelines

Education Research and Practice Development

The postholder;

Ensures that staff appraisals are conducted annually for all staff using KSF framework under your responsibility

Participates in, supports and encourages research and evidence based practice for the enhancement of patient care.

Identifies own learning needs, Is responsible for ensuring that their own continuing professional development is in keeping with Prep requirements.

Encourages education and practice development and creates an environment that encourages reflective practice through action learning, clinical supervision and peer review. .

Ensures orientation and induction programmes are in place for new members of staff.

Contributes to the development of a learning culture within the Inclusion Health Service.

Encourages all staff to participate in lifelong learning initiatives.

Creates opportunities in the team to reflect on practice in order to continuously challenge the espoused philosophy of care and identify areas for development.

Acts as a role model for other nursing staff in the clinical area, working with them to demonstrate and promote high standards of clinical treatment and nursing care, coaching them to deliver care to these standards.

Advocate for this population in terms of equality of health care services

Address barriers to health access and challenges Policy and criteria that prohibits timely treatment and care for this group.

Management

Supports nursing staff to develop management and leadership skills through appropriate delegation, supervision and role modelling.

Participates in compiling duty rotas, annual leave and study leave to ensure adequate cover at all times

Develops a participative management approach within the Inclusion health service.

Ensures there is adequate nursing cover and appropriate skill mix.

Participate in recruitment and selection of staff.

Actively explores mechanisms for retaining staff and evaluation of same.

Observes any signs of ill health or stress factors in staff assigned to the area and reports as appropriate to the nursing services manager.

In the absence of the healthcare coordinator assists in managing sickness and absenteeism and takes appropriate action in line with Trust policy.

Ensures effective and efficient use of resources.

Demonstrates effective decision making that is patient centred and consistent with Trust policies.

Ensures systems are in place to provide a safe environment for patients' care within the risk management and clinical governance strategy.

Adheres to all Trust policies

Ensures that systems are in place for safe use of medical devices and that incidents/accidents on faulty devices are recorded, reported and investigated in accordance with Trust policy.

Infection Prevention and Control

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff.

This includes:-

Cleaning hands either with soap and water or a hand sanitizer at the appropriate times (WHO '5 moments');

Using the correct '7 step' hand hygiene technique;

Being 'bare below the elbows' when in a clinical environment;

Following Trust policies and the Regional Infection Control Manual (found on intranet);

Wearing the correct Personal Protective Equipment (PPE);

Ensuring correct handling and disposal of waste (including sharps) and laundry;

Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilized;

Ensure that you are trained and keep update in all Covid -19 guidelines

PERSONNEL SPECIFICATION

Registered Nurse (Adult) Homeless Public Health Nursing Service / Band 7

Have a Nursing Degree AND 3 years post-registration experience within acute or unscheduled care particularly working with vulnerable patients (e.g. Homeless, MH & Addictions)

Or

Have 5 years post-registration experience as a Band 5 including 3 years within acute or unscheduled care particularly working with vulnerable patients (e.g. Homeless, MH & Addictions) and have obtained a postgraduate

diploma or equivalent qualification.

Have 5 years post-registration experience which must include a minimum of 2

years at Band 6 in acute or unscheduled care particularly working with vulnerable patients (e.g. Homeless, MH & Addictions)

An Interest in improving the health and health care experience for this population.

Provides evidence of personal and professional development.

Hold a current full driving licence - have access to a car or form of transport that will enable the post holder to carry out their duties.
