An exciting opportunity is available within the Tissue Viability Service at Midlands Partnership Foundation Trust.

The successful applicant will join the senior team, offering advice and practical support to staff & patients, on the management of tissue viability issues. The provision of specialist clinical advice for individual patients includes referral management, telephone triage, remote assessment & planning, face-to-face complex assessment & recommendations. Other tasks involve asset management, audit, and lead on research & innovation. It will require dissemination of knowledge & skills involves planning & design of training sessions and e-learning packages, face-to-face training and small group and 1-to-1 coaching. The role includes internal team management responsibilities and representation at meetings in support of Trust and other objectives. The postholder will play an active role in the development of the service, the implementation of standards and associated guidelines and policy adherence.

Main duties of the job

  • The provision of high-quality evidence based person centred wound care for patients registered with a Staffordshire GP, encompassing physical, mental & social health care needs.
  • Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team.
  • Facilitating learning through the development and delivery of specialist clinically focussed education and training programmes (including health assessment, independent prescribing, long term conditions management and SPQ).
  • Being a clinical expert and autonomous practitioner specialising in the field of tissue viability practice; leading, influencing, developing and expanding the Tissue Viability service
  • Implementing appropriate internal and external reporting in accordance with contracted and national guidance to ensure compliance with best practice in the prevention of tissue damage

  • The post holder will be an expert, autonomous practitioner specialising in the field of tissue viability practice; leading, influencing, developing and expanding the Tissue Viability service throughout the Trust in all community settings.

You must have appropriate UK professional registration.

  • Evidence of promoting research, evidence based practice, clinical excellence and quality approaches to support outcome focussed care
  • Experience in facilitating learning within clinical practice
  • Experience of working with a range of statutory and non-statutory agencies
  • Experience of the administration and management of medicines according to policy and local protocols

Desirable

  • Service improvement experience

Skills & Knowledge

Essential

  • Skills in assessing, planning and evaluating physical health care/treatment interventions
  • Practices within the four pillars of advanced clinical practice: clinical practice, leadership and management, education and research
  • Advanced clinical, theoretical and practical knowledge across a range of work procedures relevant to the speciality of wound care
  • Skills in co-ordinating complex care including leading MDT meetings
  • An ability to work independently and within a team

Qualifications & Training

Essential

  • Registered Nurse (Adult Nursing)
  • Advanced physical healthcare assessment qualification
  • Independent and supplementary prescribing qualification
  • Post graduate qualification in Tissue Viability or Wound Management
  • Masters level qualification in Advanced Clinical Practice
