The Advanced Clinical Practitioner is an autonomous senior clinical leadership role. The post holder will have undertaken post-graduate training in Advanced Clinical Practice, while having consolidated your advanced-level knowledge and skills within your scope of practice. You will have gained a high-level of training and practice-based experience through the four pillars of advanced practice (Clinical Practice, Leadership, Education and Research).

This position is located at HMP Wandsworth the post holder will be expected to work on site in the prison.

Main duties of the job

Key Task and Responsibilities The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care, ensuring consistently high standards of practice and clinical excellence, including: promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives.

The post holder is expected to place the well-being and safety of service-users/patients above all other considerations, with responsibility to recognise and work within the limits of your professional competence.

The post holder will be part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, and prison managing a single referral pathway.

Detailed job description and main responsibilities

Management & Leadership responsibilities

• Demonstrate a deep and systematic knowledge and understanding of local care systems (including agencies outside of statutory care); collaborating with and referring to them independently and when required.
• Act as clinical lead on the delivery of specialist-specific interventions, working in collaboration with the person, their family and carers, as well as a range of professionals and agencies.
• Demonstrate support, respect and courtesy towards colleagues; working in a professional manner and with Trust values always in mind, respecting the boundaries of the relationship with the person, carers and staff.
• Challenges, where appropriate, professional and organisational boundaries, identify areas for skill/knowledge development and apply these to practice to ensure continuity and high standards of care.
• Exemplifies leadership, resilience and determination, manage situations that are unfamiliar, uncertain, complex or unpredictable and seeks to build confidence in others.
• Receives, leads, delivers and exemplifies a culture of critically reflective clinical supervision.
• Manages the expectations of both clinical interventions and service demands in knowledge of available resources (i.e. finance, staffing and access to services).
• May manage less experienced staff; junior clinicians, trainees and support workers.
• Supports the development of trainee advanced clinical practitioners, acting as a role model, supervisor and assessor.

Clinical

• Demonstrates the advanced skills and knowledge to undertake comprehensive person-centred history taking, assessment and formulation; considering biological, psychological, social and spiritual perspectives.
• Critically assesses and determine a person’s capacity to make choices and decisions.
• Uses expertise and decision-making skills to appraise complex, incomplete, ambiguous and conflicting information, synthesizing information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
• Critically applies care and treatment in line with national and local guidance; utilising evidence-based interventions within scope of practice, referring people to other professionals or services as the need requires.
• Demonstrates the ability to prioritise interventions in situations where there are multiple issues, changing priorities and/or safety concerns, especially in the presence of multi-morbidity, complexity and/or unpredictability.
• Demonstrates the critical use of evidence-based evaluation tools and/or outcome measures to assess the efficacy of interventions; collaboratively making appropriate care planning modifications with the person, carers and other relevant parties.
• Demonstrates an in-depth critical understanding of the principles of therapeutics, polypharmacy, de-prescribing, optimal prescribing, adverse and toxic medication effects and medication burden.
• Identifies and leads response, in a timely manner, to the condition and presentation of the person; in the presence of physical health acuity and/or mental health deterioration.
• Keep accurate, contemporaneous documentation and care plans, in line with clinical governance and organisational policy.

Research

• Demonstrates critical awareness and experiential knowledge on a range of methodological approaches in leading, delivering and evaluating change through research and quality improvement.
• Undertakes clinical care audits; participates in and co-ordinates one or more formal research programmes within clinical specialism/service setting.
• Develops and implements robust clinical governance systems to enhance your practice and the practice of others.
• Interprets national professional policies for specialist area, remaining up-to-date with contemporary evidenced-based practices relevant to scope of practice; disseminating information and guidance to others within and beyond the immediate service.
• Contributes to the evidence-base within field of practice through internal and external dissemination of knowledge (i.e. writing for publication and delivering conference presentations on research/QI).

Communication

• Demonstrate the knowledge and skills to articulate comprehensive assessment, formulation and care planning, verbally and in written form.
• Use advanced communication skills to exemplify an attitude of hope, optimism and partnership with the person, carers and their loved ones.
• Act with inclusivity, initiative and creativity to incorporate strength-based principles to support people to address their individual needs.
• Communicates specialist condition-related information to the person, carers and other health professionals; where there may be significant barriers to acceptance or understanding.
• Uses communication, negotiation, conflict management skills; requiring empathy, reassurance and persuasion, where there are significant barriers to change.
• Demonstrates critical understanding of how the person’s and professional’s values and perspectives may differ and the influence this may have on engagement, assessment and treatment.
• Demonstrates a comprehensive critical awareness of the potential for stigmatisation and the possible impact on co-production, intervention and motivation.
• Demonstrates a depth of understanding around the impact of population health and wellbeing, taking appropriate steps to address inequalities and vulnerabilities.
• Demonstrates a critical understanding of the multiple perspectives within a situation; ensuring the needs of carers, of all ages, are appropriately assessed, explored and addressed.

Custodial Responsibilities • Assume personal responsibility for the security of issued keys.

• Understand and comply with prison orders, procedures, and instructions in your area of work.
• Comply with all security requirements.
• Respond to any situation or circumstance that might indicate a threat to security of the establishment or to the safety of an individual, completing Incident, Security, Injury, or other reports as appropriate.
• Report breaches of order and discipline including reporting and recording untoward incidents according to local protocol.
• Contribute to effective risk assessment and management procedures.

IMPORTANT INFORMATION, PLEASE READ:

All applicants must be willing to undertake National Security Vetting in order to work in a Prison Setting. This will be completed as part of the pre-employment checks through Oxleas and the prison vetting team.

Proof of ID, needs to include 1 photographic ID

Proof of address documentation

Non-UK passport holders will need to have correct documentation (right to work in the UK) and a Home office Share code.

Address History:

5 years address history will be needed.

Applicants that are not UK Passport holders who provide less than 5 years UK address history will need to provide a Police Certificate which must be in English from where they resided previously.

Applicants who are UK Passport holders who have lived abroad for a period of more than six months during the last three years will need to provide a certificate of good conduct or an overseas police check in English from the countries resided in or visited.
