We are looking for an enthusiastic Ward Clerk to join our Community Ready Unit (CRU), a busy and innovative service that supports the safe and timely discharge of patients from hospital. As the first point of contact for patients, relatives and healthcare professionals, you will play a vital role in keeping the service running smoothly while supporting patient flow across the Trust.
The Community Ready Unit is at the heart of discharge and flow, helping up to 50 patients each day transition safely from hospital to home or onward care. Working alongside nurses, therapists, doctors, pharmacy teams, transport providers and community partners, no two days are ever the same.

Main duties of the job
The Ward Clerk will provide comprehensive administrative support to the Community Ready Unit and act as a key point of contact for patients, relatives, carers and healthcare professionals. The post holder will maintain accurate patient records, support admissions, transfers and discharges, manage telephone enquiries and ensure patient documentation is completed and filed appropriately.
The role includes coordinating transport arrangements, liaising with wards, departments, community services and external agencies, and supporting the multidisciplinary team to facilitate safe and timely patient discharge. The Ward Clerk will handle confidential information in accordance with Trust policies, maintain ward supplies and stationery, support the safe keeping of patient valuables and contribute to the smooth day to day running of the unit.
The successful candidate will work within a busy patient facing environment, with direct contact with Patients/Families and Professionals, communicating effectively with patients and families, including those who may be vulnerable or distressed. They will support discharge coordination processes, escalate safeguarding or welfare concerns where appropriate and contribute to maintaining a safe, organised and welcoming environment for patients and staff.
The post holder will be expected to work flexibly, manage sensitive information, participate in mandatory training and development and uphold the values of the Trust.
