The Virtual Ward forms part of the Urgent Community Response (UCR) team. We are a 7-day service that supports acutely unwell patients that would otherwise be in a hospital bed, supporting both early discharge from hospital and admission avoidance. The team responds to the needs of patients in the place they call home.
The service provides both in person care from nursing, therapies and social support, as well as remote monitoring of vital signs and facilitation of remote consultations and communication with patients.
We have an exciting opportunity for an experienced nurse, paramedic, allied health professional (AHP) with a passion for new ways of delivering patient care. You will possess the drive, enthusiasm and talent to develop services to support patients on the Virtual Ward and lead the coordination and remote monitoring team based within our Virtual Ward Hub. We are seeking someone who wants to develop their skills in leadership and develop a service at the forefront of healthcare delivery.
The post holder will be an experienced Registered Nurse, HCPC registered Paramedic or AHP who will hold a key position within the Virtual Ward team. Main duties of the job
As a Band 7 Virtual Ward Clinical Coordinator you will:
- Act as the lead within the Virtual Ward Hub and support the development of the service by leading on projects and audits with the heads of service.
- Lead the clinical coordination of the team on shift. Oversee and lead the clinical triage of referrals, onboarding and clinical management for patients on the Virtual Ward.
- Support the team to develop their clinical skills in conducting remote clinical assessments and treatment for patients across the Virtual Ward pathways.
- Maintain clinical skills working regularly as part of the Virtual Ward Hub delivery team.
- Create a ward culture that values and supports staff to reach their full potential, with a high degree of personal and professional satisfaction.
- Facilitate optimum standards of care, initiating, implementing and evaluating programmes of ongoing improvement within a positive learning culture.
The role will be primarily office based within the Virtual Ward Hub. At present the role is core working hours 08:00 - 18:00 across 4 days, with weekend/bank holiday working on a rota basis. The Virtual Ward core clinical team is integrated with Urgent Community Response (UCR) and works 7 days a week covering shifts between 8:00 until 22:00.

Skills
Essential
- Able to demonstrate effective leadership, excellent interpersonal skills and organisational skills
- Advanced clinical decision making skills to support patient centred care
- Knowledge of Safeguarding and the principles of Adult & Child protection
- Sound understanding of clinical governance
- Evidence of ability to maintain and monitor high standards of care
- Ability to support staff, delegate and organise workload and refer to appropriate services/agencies
- Excellent IT skills including knowledge of Microsoft Office – Word and Excel as well as clinical documentation systems
Desirable
- Demonstrates a sound understanding of community developments and priorities; locally and nationally
- Ability to influence and manage a variety of change management programmes in embracing and implementing change to provide high quality health outcomes
Experience
Essential
- Experience of working at a Band 6 level in either an acute or community environment
- Experience leading and supporting a team
- Multi-disciplinary and multi-agency working
Desirable
- Working knowledge of East Sussex community services
Qualifications
Essential
- Registered health care professional with an accredited health profession
- Advanced Physical assessment module or equivalent
- Level 6 Degree or evidence of working towards in relevant subject
- Teaching and Mentoring qualification (or relevant experience to undertake training within 12mths of coming into post
Desirable
- Evidence of continuing professional development in a relevant area of specialist practice to Masters equivalent level
