Job overview

The Band 6 Urology Clinical Nurse Specialist will provide specialist nursing care and act as key worker for patients with urological cancers.

The post holder will coordinate and facilitate cancer treatment pathways, deliver and monitor agreed cancer therapies within established competencies, undertake nurse-led clinics, support renal cancer and nephrostomy services, attend Patient Tracking List meetings, participate in multidisciplinary team working, and provide expert advice regarding treatment options and treatment-related side effects. The post holder will contribute to Holistic Needs Assessment, Personalised Care and Support Planning, cancer pathway monitoring, governance, audit and service improvement activities, BCG and Mitomycin administration. Prostate cancer hormone therapy clinics. ensuring patients receive high-quality, personalised care throughout their cancer journey.

• Working towards advanced assessment and non-medical prescribing competencies where applicable.

Detailed job description and main responsibilities

• Coordinate and monitor the patient's cancer journey from diagnosis through treatment and follow-up.
• Facilitate referrals for investigations, treatments and specialist services.
• Arrange and support follow-up cancer appointments after MDT discussions.
• Ensure timely communication with GPs and other healthcare providers, including serious diagnosis notifications
• Act as the key worker for patients diagnosed with urological malignancies, providing continuity of care throughout the patient's cancer pathway.
• Coordinate patient journeys from diagnosis through treatment, follow-up, surveillance and survivorship.
• Ensure timely access to investigations, multidisciplinary team (MDT) discussions, treatment planning and appointments.
• Liaise effectively with urologists, oncologists, radiologists, chemotherapy services, primary care teams and allied health professionals to facilitate seamless patient-centred care.
• Attend and actively contribute to Patient Tracking List (PTL) meetings and cancer pathway meetings, ensuring timely progression of patients through diagnostic and treatment pathways.
• Monitor cancer waiting time pathways and escalate actual or potential breaches that may impact treatment delivery or patient outcomes.
• Communicate MDT outcomes and treatment plans to patients and relevant healthcare professionals in a timely manner.

Administration and Monitoring of Cancer Treatments

• Assess patients before treatment, identifying contraindications, treatment-related risks or concerns requiring medical review.
• Deliver and monitor agreed cancer treatments within scope of practice, competency and local governance frameworks, including: Intravesical Bacillus Calmette-Guérin (BCG) therapy. Intravesical Mitomycin-C chemotherapy. Prostate cancer hormone therapy injections. Nurse-led cancer treatment reviews where locally approved.

• Monitor patients for treatment-related toxicity, adverse reactions and complications, taking appropriate action as required.
• Provide treatment-specific education, counselling and informed support to patients and carers.

Treatment Follow-up and Toxicity Management

• Provide specialist telephone advice and support for patients receiving cancer treatments.
• Assess, triage and manage treatment-related symptoms and side effects in collaboration with medical and oncology colleagues.
• Escalate concerns relating to treatment toxicity, sepsis, disease progression, treatment intolerance or psychological distress.
• Coordinate surveillance investigations, follow-up appointments and treatment reviews following completion of therapy.
• Ensure patients receive timely referral to appropriate support services when required.

Nurse-Led Clinics

Manage autonomous and consultant-supported clinics including:

• Newly diagnosed urological cancer patients.
• Patients receiving active cancer treatment.
• Prostate cancer hormone therapy monitoring and review.
• Surveillance and follow-up clinics.
• Holistic Needs Assessments (HNA).
• Personalised Care and Support Planning (PCSP).
• Supported self-management and survivorship pathways.
• Cancer Care Reviews in collaboration with primary care services.

Renal Cancer and Nephrostomy Service Support

• Support Band 7 Clinical Nurse Specialists in the management of renal cancer pathways and complex urology patients.
• Assist in the assessment, review and ongoing management of patients with nephrostomies, ensuring complications are promptly identified and escalated.
• Undertake nephrostomy dressing changes and associated care following completion of agreed competency assessments and in accordance with Trust policies and local procedures.
• Participate in patient education relating to nephrostomy care and ongoing symptom management.
• Contribute to service delivery across renal and cancer pathways to ensure safe, effective and timely care.

Governance, Quality Improvement and Service Development

• Participate in cancer MDT meetings and contribute specialist nursing expertise to treatment planning and decision-making.
• Maintain accurate cancer pathway tracking data and support achievement of national cancer standards and targets.
• Participate in audit, quality improvement, service development, research and service transformation initiatives relating to urological cancer services.
• Contribute to Systemic Anti-Cancer Therapy (SACT) governance processes and ensure compliance with Trust policies and national guidance.
• Support the development, implementation and evaluation of evidence-based pathways, standards and standard operating procedures.
• Maintain professional competence through continuing professional development and participation in relevant training programmes.
• Provide emotional, psychological and practical support following a cancer diagnosis.
• Signpost patients to Macmillan, financial advice services, support groups and community resources. Develop and review patient information leaflets and educational resources. Provide information prescriptions and support informed decision-making
• Act as a specialist resource for ward and outpatient staff.
• Provide education and training to nursing and multidisciplinary colleagues.
• Support service development and implementation of cancer care pathways. Maintain specialist knowledge through continuing professional development.
