Responsibilities include leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and completing them. Other duties include assisting in the smooth transition of discharges and monitoring the well-being of each resident. Practicing safe systems of work and moving and handling of residents are also key responsibilities. Our ideal candidate must have an NVQ 3 or equivalent in care for care homes, two years experience in a senior position with the resident group, and be able to communicate effectively.

ABOUT THE ROLE

- Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
