Job overview

As a member of the Stroke team, the post holder will take a key role in developing the stroke service across the organisation.

The post holder will ensure all acute stroke patients receive timely access to the stroke pathway 24/7 in line with agreed best practice to ensure compliance with Stroke Sentinel National Audit Programme (SSNAP) and RCP Stroke Guidelines.

The Band 6 stroke nurse will be called, anytime of the day, should a patient present in the Emergency Department (or another location within the hospital), with acute stroke/TIA. They will support the smooth running and co-ordination of the 7 day a week TIA service and ensure TIA patients are managed in line with the local pathway.

The post-holder will be expected to be involved in the collection of National Stroke data. They will be involved in facilitating stroke service improvement to streamline the patient journey and improve the quality of patient care, and in developing information systems, in line with Trust strategy.

The expectations within this job description will be met through clinical management, hands-on clinical practice, professional leadership and development, education, training, analysis of Stroke care through data collection and research.
