Main duties of the job

  • Be accountable for performing regular assessments on patient needs, to plan, implement and evaluate care delivery and be adaptable to changing health care requirements.
  • You will be responsible for establishing and maintaining effective communication with patients, relatives and carers to always ensure their involvement in the decisions about treatment, planning and the delivery of their care.
  • You will take responsibility for working collaboratively with your team, other professionals and agencies to always ensure the patient’s needs are met, especially in relation to their on-going health care needs, their transfer and their discharge planning.
  • Be accountable for ensuring your daily practice fulfils your professional requirements as outlined by the NMC code of conduct and guiding documents.
  • Be accountable for maintaining your active status on the NMC register.
  • You will actively participate in patient and public involvement activities.
  • All employees have the responsibility to comply with the Trusts Infection Control and Prevention policies and procedures at all times. Strict adherence to effective hand hygiene is essential.

  • Excellent communication skills both written and verbal with the ability to overcome barriers to communication
  • Able to work as part of a team
  • Act as a facilitator and enabler with patients / carers
  • Good organisational skills and able to prioritise and delegate
  • Confident in dealing with difficult situations
  • Able to motivate and encourage others

Desirable

  • Evidence of supporting and teaching colleagues
  • Experience of working closely with external providers to ensure that patients are directed to the right sort of support and services available to them
  • Confident in the use of NHS IT systems

Experience

Essential

  • Experience of working as part of a multidisciplinary team
  • Acute recent NHS hospital experience

Desirable

  • 6 months post-registration experience
  • Experience of involvement with discharge planning
  • Evidence of working as part of a wider MDT

Qualifications

Essential

  • Registered Nurse with a diploma/degree

Desirable

  • Mentor Qualification
  • Evidence of up-to date mandatory training.
  • Evidence of continued personal professional development.
