As a Community Nurse at the Virtual Ward you will be expected to manage daily operations of caseload, triage referrals for admission, support staff with patient escalation. Troubleshoot, safety net and make clinical decisions to keep patients safe at home. In addition, you will further develop junior staff. Proficiency in IT and the ability to quickly learn multiple computer systems are crucial for this position.
Please note this vacancy role requires face to face patient management as well as virtual management and therefore the team members will be expected to conduct face to face assessments in line with patient/ caseload needs. We are particularly interested in nurses with a surgical background as we look to develop our surgical pathways.
Main duties of the job
To provide high quality nursing care and participate in the assessment, planning, implementation and evaluation of the care needs for the patient group.
To deliver compassionate person-centred care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.
To provide relevant forms of care with minimal supervision and participate in the supervision and teaching and assessment of unregistered or junior staff and students.
To play an active part in the continuing development of the ward or department and contribute to the maintenance of an environment conducive to the delivery of high standards of patient care, by promoting good communication and relationships between staff, patients, relatives and visitors.
Providing IV medications to patients with a home environment and clinic/outpatient setting to avoid hospital admission.

- Strong IT skills

- Degree module for long term conditions ie respiratory, asthma, diabetes
- Health assessment module
Registered Nursing experience of at least 6-12 months

- NMC Qualification
- FULL UK Driving license and access to own vehicle with business insurance (cover for driving for work)
- Registered Nursing experience of at least 6-12 months
- Competent in IV administration
Desirable
- Competent in use of longlines (PICC, Hickman, smart-mid)
- Cannulation and venepuncture
