This post is based at the Northwick Park satellite site within the Northwest Directorate. Moorfields is one of the world's leading eye hospitals, providing expertise in clinical care, research and education.

The post holder is responsible for assessment of care needs of patients, planning programmes of care, implementation and evaluation of these programmes with direct supervision. At times they may have management responsibility for being in-charge of the department and supervising junior members of staff. The post holder will be a key member in relation to the development of practice in the department.

They will be working independently/autonomously and at times without direct supervision, accountable for their own professional actions. Ensuring the safety and well-being of service users, and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.

Main duties of the job

• Ensuring that high quality, current evidence-based nursing assessment, care planning, implementation, interventions and evaluations for patients is provided from admission through to discharge.
• Be responsible for the application of knowledge across the range of ophthalmic work procedures and practices, underpinned by theoretical knowledge and practical experience.
• To act as advocate for patients within the area, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
• Recognise the significance of observations made, and use them to develop and initiate nursing assessments and devise a plan of care.
• Carry out those activities necessary to conduct a comprehensive assessment of a person’s nursing requirements.
• To take a clear and concise history from patients by assessing their health and well-being and complete documentation ensuring that all entries are accurate and legible and that all information systems are maintained.
• To work in Day Care, ensuring the surgical pathway is running smoothly and on the day cancellations are prevented.

Detailed job description and main responsibilities

Clinical and Patient Care:

• Ensure safe peri operative care through effective admission and discharge planning.
• To support qualified staff in the assessment of health needs, planning, delivery and evaluation of care for a number of individuals or groups of patients, including clinical risk assessment.
• To exercise a reasonable and agreed degree of professional autonomy and make judgements and decisions in order to satisfy the expectations and demands of the job.
• To undertake clinical work and related tasks as delegated by qualified nursing staff following appropriate training and assessment of competency.
• To liaise effectively with all stakeholders in the care process, including carers and relatives, aspiring to provide an effective, seamless and integrated service to patients.
• To attain competencies, by following the Trusts required protocol. · To act as chaperone when required.

Professional Responsibilities:

• To maintain and update relevant knowledge and skills, maintain a professional portfolio and participate in further training as appropriate, in order to enhance quality of care for patients and carers.
• To undertake clinical supervision in line with Trust Policy, to further develop competence and clinical practice.

• To communicate effectively and in a timely manner within the team.
• Understand the principles of confidentiality.
• Always communicate in a way that maintains the patient’s confidentiality.

• To develop within the framework of a personal development plan agreed with your manager.
• To participate in training to achieve the competencies required to undertake the role under the supervision of a trained nurse.

• Maintain clear and legible patient records in accordance with both Trust and professional standards for record keeping.
• In conjunction with the Manager and other senior members of the multi-disciplinary team support a positive culture and environment for both staff an patients.
