We are seeking a motivated and experienced Band 6 Specialist Patient Flow Coordinator to join our specialist team, this is a  permanent post available.

Working autonomously across Cardiothoracic, Critical Care, Nephrology and Transplant, Neurosciences and Haematology directorates, the post‑holder will play a key role in ensuring safe, timely and effective patient flow into secondary and tertiary services.

The role requires close collaboration with senior nurses and Clinical Board senior support, acting as a key decision‑maker in the allocation of beds and resources. The successful candidate will demonstrate strong leadership skills, supporting ward teams to maintain robust processes that promote efficient discharge planning, minimise delays and optimise service capacity. The ability to speak Welsh is desirable; Welsh and English speakers are equally welcome to apply.

Main duties of the job

We have an opportunity to appoint a permanent Band 6  Specialist Patient Flow Coordinator to strengthen patient flow across Cardiothoracic, Critical Care, Nephrology and Transplant, Neurosciences and Haematology directorates.

Post‑holders will work both autonomously and as part of the specialist team, collaborating closely with the Clinical Board senior support to ensure timely and effective flow into secondary and tertiary services. They will act as key decision‑makers, working alongside senior nurses to ensure appropriate allocation of resources and supporting ward teams to embed robust processes that promote safe, efficient and appropriate patient flow.

Strong leadership skills are essential, and the ability to speak Welsh is desirable; Welsh and English speakers are equally welcome to apply.
