Are you a skilled diabetes nurse looking for a new challenge?

As the Specialist Diabetes Nurse within the community diabetes team, you will work alongside the Specialist Diabetes Dietitian to provide leadership to our friendly and supportive team of nurses, dietitians, nutritionists and administrators who together deliver care to people with complex type 2 diabetes.

Working across the county of Gloucestershire, you will deliver care to people within their own homes and in clinic settings, helping people with diabetes to feel empowered to effectively self-manage their condition.

You will also work closely with colleagues across the healthcare system, helping to develop integrated pathways for care and enhancing skills in diabetes management across the workforce

Main duties of the job

To work effectively as a member of the MDT, which includes consultants, diabetes specialist nurses and dietitians, nutritionists, primary care colleagues, community teams and medical staff to improve outcomes in patients with type 2 diabetes.

To provide highly specialist advice on diabetes management to enable the delivery of evidence-based diabetes care.

To contribute to MDT clinics in primary and secondary care.

Provide joint practice-based clinic sessions to optimise diabetes treatment care planning and prescribing decisions for complex patients within primary care.

To support the development of integrated pathway working across organisational boundaries.

To share expertise in diabetes management within the diabetes team, organisation and wider local healthcare system.

To provide clinical leadership and line management to nurses in the diabetes team.

To work collaboratively with system colleagues to develop and update local strategy, guidelines and protocols for the management of type two diabetes in Gloucestershire, which will impact on the practice of other teams.

Detailed job description and main responsibilities

We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module “Preceptorship for Healthcare” (AP7026).

Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.
