Are you interested in working as part of a friendly, well- established community rehabilitation team, supporting therapy services to adults with a stroke diagnosis within the community?

Lewisham Community Therapies stroke team has a vacancy for a Band 4 six-month stroke review post. You would be part of a supportive multidisciplinary rehabilitation team joining physiotherapists, occupational therapists, speech and language therapists and rehabilitation assistants, and a clinical psychologist.

Main duties of the job

The postholder will complete six-month stroke reviews either virtually or in the patients’ homes as outlined in the National Stroke guidelines

The postholder will be responsible for assisting the Community Stroke Team with their daily administration and clerical duties. This includes data base management (SSNAP- Sentinel stroke audit programme) and data collection and completing six month stroke review reports.  We are looking for an individual with excellent communication skills, IT skills and good organisational skills.  The role will also include supporting stroke team with delivery of small pieces of equipment and supporting therapists to complete therapy practice as needed.

Detailed job description and main responsibilities

  • To undertake all aspects of the six month stroke review duties, including planning home visits, undertaking blood pressure, maintaining databases and completing reports.
  • To provide stroke education and information, to relatives and carers, disciplines and agencies.
  • To participate in multidisciplinary / multi-agency team meetings and case  conferences, to ensure the co-ordination of patient care. This may include the review of patient progress and discharge planning.
  • To maintain accurate clinical records and update databases relating to the six month stroke reviews.
  • To complete the standardised stroke review questionnaire and to summarise the information in written format to the patient and their GP.
  • To produce patient letters and reports for GP’s and Stroke Consultants.
  • To input to databases as required, including the SSNAP database.
  • To carry out therapy assistant duties as required
