Job summary

Our Single Point of Contact team based to Fort Southwick, Fareham, are an essential first point of contact for those referring into our community services across Fareham, Gosport and South East Hampshire. It is thanks to this team that our patients, GPs and carers, can access the professional healthcare support they can expect.

If you come to work with us, you will work with the team

- to cover the Single Point of Contact referral service, taking referrals from clinicians and patients requesting community services intervention and support to prevent hospital admission and for identified single service referrals as agreed

- to complete the necessary referral form accurately with appropriate checks to present to health professionals with full, up to date information prior to patient assessment

- to provide complex information and use knowledge gained to liaise with a variety of health professionals including GP's, to identify and co-ordinate the appropriate action plan for the patients

- to negotiate with multiple therapies/agencies and appropriate speed of response for completion of multi-disciplinary assessment of patient needs

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties of the job

To cover the Single Point of Contact referral service taking referrals from clinicians and patients requesting community services intervention and support to prevent hospital admission and for identified single service referrals as agreed

To complete the necessary referral form accurately with appropriate checks to present to health professionals with full, up to date information prior to patient assessment

To provide complex information and use knowledge gained to liaise with a variety of health professionals including GP's, to identify and co-ordinate the appropriate action plan for the patients

Negotiate with multiple therapies/agencies and appropriate speed of response for completion of multi-disciplinary assessment of patient needs

To act as an information point of contact for professional callers. Taking and recording accurate telephone messages, seeking out and relaying information

To update patient records electronically on Rio and maintain databases/spreadsheets using office protocols

To provide full secretarial and administrative services when required

To set up and maintain comprehensive filing and patient record systems if necessary -- to ensure easy retrieval of information

To ensure incoming post is opened or distributed to the appropriate teams

To arrange meetings, training events, accommodation, and presentation equipment when necessary

To maintain adequate stocks of all necessary day to day stationery items

Experience

Essential

  • *Working knowledge of clerical/office procedures
  • *Good understanding of confidentiality and data protection
  • *Ability to communicate with professionals and managers at all levels both verbally and written

Desirable

  • *Experience within the NHS or patient centred organisations
  • *Ability to create and maintain databases

Qualifications

Essential

  • *Educated to GCSE or O level or equivalent including Maths and English grade C or above
  • *NVQ level 3 in Business and Administration or equivalent.
  • *IT experience in the user of computer software packages (Microsoft office)
  • *RSA III or equivalent audio typing

Desirable

  • *Prior training on systems used by HIOWH or other organisations
  • *AMSPAR level 2 in Medical Terminology
  • *IT qualification/training evidence
