The post holder will need to work flexibly to meet the needs of the service with some evening working.

JOB SUMMARY

The post holder will co-ordinate the planning, delivery and monitoring of evidence-based behaviour change programmes to reduce the incidence of long-term conditions in those at higher risk. This will include managing delivery of a licensed Diabetes Prevention programmes for those at higher risk of Type 2 Diabetes. The post holder will manage a team of Health Coaches and administrative support to deliver, monitor and evaluate online and face to face Diabetes Prevention Programmes. The post holder will work closely with Primary Care Teams; Health Improvement leads and other Trust staff to embed referral mechanisms and post programme signposting and support.

The post holder will support the Assistant Director for Promoting Wellbeing and Heads of Service in line with regional strategic priorities outlined in ‘Health and Wellbeing 2026: Delivering Together, Programme for Government, Making Life Better and local priorities outlined in the Trust corporate plan and local government Community Planning processes.

KEY DUTIES / RESPONSIBILITIES

Service Delivery, Development and Evaluation

To lead a staff team to deliver, monitor and evaluate an evidence-based Diabetes Prevention Programme.(DPP)

To build and develop relationships with primary care teams to ensure that patients are referred to the Diabetes Prevention Programme.

To oversee the use of the encompass digital system for receipt and management of referrals, programme delivery, monitoring and reporting in line with Trust policies and procedures and data protection requirements.

To ensure effective systems are in place to monitor and evaluate service user experience and outcomes from participating in the diabetes prevention programme.

To prepare quarterly reports, position papers and other documentation that may be required in line with programme outcomes or key performance indicators.

To Identify opportunities for meaningful involvement of service users in relation to the delivery and development of the programme.

To develop processes for on-going health and wellbeing support to patients following completion of programmes including signposting tools, pathways and peer support.

To provide leadership in service improvement and development to enhance early intervention approaches.

To support promotional activity and participate in events to promote diabetes prevention and awareness and uptake of the programme

Staff Management and Develop Team Performance

To manage the staff team including recruitment, induction, training, appraisal and management of staff in line with HSC Trust policies.

To lead the team and ensure effective team working to deliver a high-quality service to promote health and wellbeing.

To assess staff training needs and ensure staff team are trained to deliver the Diabetes prevention programme effectively, adhering to licensing, NICE guidance and other relevant quality frameworks.

To co-ordinate access to staff training to enable the development of complementary health improvement skills. e.g. motivational interviewing

To ensure processes are complied with to manage and take appropriate action in line with

15.To maintain a commitment to personal development, updating knowledge and skills and extending experience and expertise in areas relevant to the job remit.

Performance Monitoring

16. Ensure best use of resources and that the service is managed in accordance with yearly financial instructions.

17. To maintain monitoring systems that facilitate timely review of service users, record keeping and on-going reporting and review of programme delivery.

18.To develop and maintain robust project management systems including risk assessment and management.

19. To actively participate in audit activities within the diabetes prevention team and present findings as required.

20. To facilitate the evaluation of the programme ensuring follow up tracking of patients post programme to ensure relevant data collection is undertaken and collated.

Communication and Information Management

21.To ensure compliance with information governance in processing of GP referrals, managing patient records, consent and data in line with Trust policies and procedures.

22. To prepare quarterly reports, position papers and other documentation that may be required in line with programme outcomes or key performance indicators.

23. Work in partnership with manager to develop proposals for commissioners or funders for service improvements or to secure new resources.

24. Represent the interests of the Trust on appropriate regional, area and local groups.

25. Any other duties within the general scope of the post and as agreed with the Assistant Director for Promoting Wellbeing.
