JOB DESCRIPTION

JOB SUMMARY

The postholder will be responsible to the Head of Hospital at Home and Intermediate Care services for the management and performance of the Intermediate Care Service. The teams are comprised of multi-disciplinary staff including physiotherapy, occupational therapy, social work, rehabilitation support workers and administrative staff.

They will play a key role in transforming services for older people in line with the principles and model for health and social care contained in the Bengoa Report and Health and Wellbeing 2026: Delivering Together. They will be responsible for the operational management of all services within the service.

The jobholder will ensure clear and effective access and care pathways are in place and, to that end, will liaise closely with a range of stakeholders. They will be responsible to the Head of Service for all aspects of the team’s performance related to agreed key performance indicators.

ACCOUNTABLE TO Assistant Director of Enhanced Services

KEY DUTIES / RESPONSIBILITIES

Service Delivery, Quality and Governance

1. Responsible for promoting, managing the performance and future development of Intermediate Care Service and ensuring robust processes are in place to support the delivery of high quality and safe care to meet all requisite standards.

2. Ensure there are robust access and care pathways to support the co-ordination of person centred care and integrated working.

3. Ensure the effective implementation of a case management approach with all key worker and team roles clearly understood within own and collaboratively with other teams.

4. Liaise closely with directorate and other Trust colleagues, general practitioners and a range of other statutory, independent and voluntary agencies to maximise the independence of patients and clients, prevent unnecessary hospital admissions and facilitate early hospital discharge and referrals from transitional care.

5. Liaise and work in partnership with hospital staff, GP’s and other relevant stakeholders to develop and implement the pathway for identification of clients suitable for the Intermediate Care service.

6. Ensure that multi-professional assessments are person-centred and completed in a timely manner.

7. To discuss with the multidisciplinary team the provision of treatment within the community setting in partnership with patients and carers, statutory and voluntary agencies.

8. Evaluate the outcomes of treatment programmes in achieving optimal outcomes for patient/carers.

9. Evaluate service delivery against agreed key performance indicators and ensure that all staff are aware of monitoring process.

10. Ensure effective arrangements are in place for the identification, assessment and management of risks.

11. Ensure the processing and management of complaints, contract compliance, incidents and SAI comply with the Trust’s policy and procedures and are underpinned by transparency and a culture of continuous improvement.

12. Ensure effective implementation of the Trust’s Policy on Professional and Operational Management Interfaces within the Intermediate Care

13. To practice within the relevant Code of Professional Conduct and to maintain all team member practices under the relevant professional guidelines and regulatory body.

14. Keep abreast of changing government policies, targets and legislation relating to Health and Personal Social Services and national developments in relation to the care of older people, e.g. NSF for Older People, Transforming Your Care.

Service Planning and Modernisation

15. Support the Head of Service by leading all service planning and modernisation initiatives within the Intermediate Care Service. This will include identifying opportunities for more effective and efficient ways of working to meet identified needs and ensuring the effective implementation of such initiatives.

16. Participate in change management to meet the requirements of service provision and agreed strategies within the Reform, Modernisation and Efficiency Agenda.

17. Support the Head of Service to develop more effective partnership working with families, carers and other agencies in line with the Trust’s corporate commitment to Personal and Public Involvement (PPI).

18. Participate in research, evaluation of projects, quality initiatives and developments and facilitate staff undertaking research and audit projects. To participate in any requirements under Clinical and Social Care Governance.

19. Make an active contribution to local and regional working groups and project teams to include the development of project plans and as required lead on the work of any project subgroups.

20. To network with appropriate professionals to share innovation, developing self and others in the area of care of older people.

Leadership and People Management

21. Provide clear leadership and ensure the Intermediate Care Service has a highly skilled, flexible and motivated workforce.

22. Ensure all people management practices within the Intermediate Care Service are integrated and support a culture of effective team working, continuous improvement and innovation.

23. Ensure the effective implementation of all Trust HR/People Management policies within the team and the achievement of all relevant targets such as those relating to the completion of personal development plans, management of sickness and absenteeism etc.

24. Ensure all staffing resources are deployed to maximum benefit.

25. Support the Head of Service with all workforce planning and reform initiatives.

Financial and Resource Management

26. Manage the budget for the Intermediate Care Service and deliver the expected service and budget agreement. This will include both salaries and goods and services budget.

27. Ensure the effective implementation of all Trust financial policies and procedures within the team.

28. Ensure the effective management, use and maintenance of all physical assets within each team.

HUMAN RESOURCE MANAGEMENT RESPONSIBILTIES

1. Review individually, at least annually, the performance of immediately subordinate staff, provides guidance on personal development requirements and advises on and initiates, where appropriate, further training.

2. Maintain staff relationships and morale amongst the staff reporting to him/her.

3. Review the organisation plan and establishment level of the service for which he/she is responsible to ensure that each is consistent with achieving objectives, and recommend change where appropriate.

4. Delegate appropriate responsibility and authority to the level of staff within his/her control consistent with effective decision making, while retaining overall responsibility and accountability for results.

5. Participate, as required, in the selection and appointment of staff reporting to him/her in accordance with procedures laid down by the Trust.

6. Take such action as may be necessary in disciplinary matters in accordance with procedures laid down by the Trust.

HUMAN RESOURCE MANAGEMENT RESPONSIBILITIES

EMERGENCY PLANNING & BUSINESS CONTINUITY RESPONSIBILITIES

To work proactively with the Trust’s Emergency planner and other internal and external stakeholders to develop appropriate emergency response and business continuity plans to ensure the service can maintain a state of emergency preparedness to respond safely and effectively to a range of threats, hazards and disruption.

PERSONAL AND PUBLIC INVOLVEMENT RESPONSIBILITIES (PPI)

Lead on and be responsible for the co-ordination of the Trust’s PPI Strategy within the Division or other sphere of responsibility. This will include supporting active engagement with user groups and the voluntary and independent sectors in the design and delivery of services.

Promote and support the implementation of the Trust’s PPI Strategy and ensure all staff are aware of their responsibilities as appropriate to their job role.

GENERAL REQUIREMENTS

Applicants must have a professional qualification, either AHP, Medicine, Nursing, Pharmacy or Social Work and be live on the appropriate professional register.

Other

Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment. This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post.
