Cornwall Partnership NHS Foundation Trust (CFT) is excited to offer an opportunity for a Senior Staff Nurse within our Bodmin Same Day Emergency Care (SDEC) and Bodmin Clinical Assessment and Treatment Unit (CATU)

Patients are admitted direct for assessment from home or ED.

Do you enjoy acute nursing and dynamic patient flow? The CATU/SDEC model is an innovative and dynamic patient pathway of which a key aspect is the assessment of the acute patient and treatment of symptoms aiming to return the patient to their home with the right support. You will be empowered to utilise a range of nursing skills (supported by training) to support patients through a clinical admission, assessment, and ongoing treatment.

You should possess commitment to team working and innovative delivery of excellent standards of care. You will need to be able to demonstrate a real enthusiasm for using extended skills and a desire for on-going professional development.

Main duties of the job

On behalf of the Ward Manager, and as the nurse in charge, liaise with members of the multi-professional team both within the department and beyond, to promote effective communication and to ensure high quality care is delivered to the patient both effectively and efficiently.

To take direct referrals to the Same day emergency Care and Community Assessment and Treatment units which assess and treat as appropriate people living with frailty. The aim of the post holder is to co-ordinate the triage, arrivals, and management of the patient group within a multidisciplinary team to support alternatives to admission.

Oversee patient flow in a dynamic setting supporting people to return to their own homes where possible which requires working collaboratively with admission avoidance colleagues in Virtual Ward and community partners to provide the right support to do so.

To be responsible and accountable for the assessment of care needs and the development, implementation and evaluation of programmes of care in this setting. Initiating assessments and treatments in a nurse led environment.

To provide leadership, support and supervision to junior colleagues and to act as a role model in the assessing, planning, implementing, evaluation and delivery of a high standard of individualised care to patients and their carers while on the ward, or when appropriate throughout the hospital.

Knowledge and abilities

Essential

  • Evidence of active involvement in developing new ways of working
  • Competence in cannulation and venepuncture
  • Up to date knowledge of current clinical and professional issues
  • Knowledge of and able to implement evidence based practice
  • Ability to adminisiter meds, including transfusion of bloods/products
  • Ability to deal with the care of terminally ill and other challenging situations

Qualifications/Experience

Essential

  • Current professional registration with NMC
  • Professional nursing qualification at degree level
  • Evidence of current and ongoing professional development
  • Staff management and leadership including HR processes
  • Postgraduate qualification in leadership or equivalent experience

Desirable

  • Experienced in successfully implementing change management strategies
