Job overview

We are excited to announce an opportunity for an experienced and innovative Paediatric Nurse to lead the Palliative and End of Life (EoL) Diana Nursing Pathway within the Newham Children’s Community Nursing Service. This role places you at the center of compassionate, community-based care, providing support to children with life-limiting conditions and their families during the most critical moments of their journey.

As the Palliative and EoL Pathway Lead, you will collaborate closely with the Core Nursing Pathway Lead and the Complex Care Pathway Lead, and work as part of a broader multi-disciplinary team that includes Nursing (Specialist School Nursing, Asthma, Epilepsy, Continuing Care) and Community Paediatric Dietitians. The Pathway Lead is expected to adopt a multi-disciplinary approach to patient care, integrating psychology and play therapy services.

You will oversee the care of palliative and EoL patients in the community, which may involve managing your own caseload while also supporting and supervising band 6 children's community nurses in delivering palliative nursing care to children and their families.

Main duties of the job

To offer expert nursing guidance and support to children with life-limiting conditions or those needing end-of-life care, as well as their families throughout the borough of Newham.

In the role of nurse lead for the palliative and end-of-life nursing pathway for children, I will serve as a knowledgeable resource for staff across all teams within the Children’s Community Nursing Service (CCNS).

I am committed to overseeing end-of-life care planning and delivering exceptional care for children in their final stages.

Collaborating with the Symptom Control team at Great Ormond Street Hospital (or other relevant organizations), I will provide advice and support to children and young people facing life-limiting or life-threatening illnesses, as well as their parents, carers, and community professionals.

I will also take on the responsibility of line management for junior team members who are delivering nursing care along the palliative and end-of-life pathway.

Additionally, I will partner with the Complex Health Needs Nursing Pathway lead and the Core Nursing Pathway lead within CCNS to ensure that children and their families receive suitable nursing care and interventions.
