Job summary

You'll support people needing rehabilitation in the community, helping them recover, stay independent and improve their quality of life. You'll work as part of a multidisciplinary team, providing high-quality, nurse-led care to patients with a range of complex needs. This senior role combines hands-on clinical practice with leadership, helping shape safe and effective services. You'll build strong relationships with patients, carers and partners, ensuring care is joined-up and person-centered

Main duties of the job

What you'll do:o Assess, diagnose and manage patient care, including treatment, referral and discharge planning o Coordinate and prioritise a caseload, supporting timely rehabilitation and recovery o Work closely with multidisciplinary teams to plan and deliver joined-up care o Provide guidance, supervision and support to junior staff and students o Contribute to service development, quality improvement and evidence-based practice What we're looking for:o A registered nurse with professional registration and up-to-date clinical knowledge o Experience of working in multidisciplinary teams and managing complex patient needs o Confidence to work independently and make clinical decisions o Strong communication and organisational skills, with a focus on patient-centred care o A commitment to learning, development and supporting others to grow We welcome applications from people of all backgrounds and experiences, and value a workforce that reflects the communities we serve

Knowledge

Essential

  • Comprehensive knowledge of relevant legislation, guidance and local initiatives and issues in relation to unplanned care
  • Working knowledge of multi-disciplinary teams
  • Evidence of Continuing Professional Development
  • Demonstrate the use of evidence-based practice

Desirable

  • Knowledge of the legal framework surrounding medications prescribed or supplied.

Experience

Essential

  • Experience of multi-disciplinary team working and working with patients with acute medical conditions
  • Demonstrate experience of utilising a variety of software packages e.g. SystmOne, E-Mail, Excel, PowerPoint and Word
  • Demonstrate experience of flexible workload
  • Management and the ability to meet deadlines
  • Demonstrate experience and competency in supervision and support of others in prioritisation of patient care e.g. junior staff, non-registered staff and students

Desirable

  • Experience in Intermediate care, demonstrating effective leadership and autonomy. Or previous experience of working in a similar post
  • Experience of the initial management, stabilisation and onward referral of patients who may self-present with more serious/life threatening conditions
  • Previous experience of producing reports or audits
  • Demonstrate previous leadership and management within a clinical setting

Qualifications

Essential

  • Registered 1st level Nurse
  • NMC Registration
  • Specialist post graduate knowledge in rehabilitation or related subject gained through formal qualification or acquired through equivalent experience
  • Mentorship Qualification

Desirable

  • Non-Medical Prescribing
