The post holder will work as part of a multi-professional team managing a caseload of patients in the community, ensuring the needs of the patients are identified and met.  Promoting the independence of patients, assisting them to maintain/or improve their own physical, psychological and social wellbeing. In addition to this to arrange timely responses overnight for urgent unplanned visits.

To ensure the provision of clinical leadership and direct line management of the team members with the support of the Senior Sister/Charge Nurse is achieved.

To ensure that the service we deliver is adaptable and resilient, commercially focused and creates innovative solutions to enhance the patient experience.

We are looking for enthusiastic Senior community nurses to join us in our community Nursing Teams  within the London borough of Sutton. You will need to be passionate about delivering exceptional holistic health care, supporting patients to remain independent and avoiding unnecessary hospital admission.

Previous experience of community nursing is essential. You will work directly with patients, their families and our multidisciplinary services to plan and facilitate the delivery of holistic evidence based care.

Sutton Health and Care is a pioneering partnership made up of the GP federation in Sutton, the London Borough of Sutton, South West London and St George’s Mental Health Trust and Epsom and St Helier hospitals. Working together, we provide community services in Sutton, caring for adults in their own homes across the borough.

We are a 7 day extended hours service; working Monday to Sunday, 7am to 7pm  including weekends and bank holidays.

Access to own vehicle  insured for use at work is essential.
