Job summary
This is an exciting opportunity to work in a fast paced and dynamic team, within the Southeast Division of Hampshire and IOW trust. We have the opportunity for a full time 37.5-hour position for a band 6 clinical lead. Contributing to the prevention of hospital admissions under a strong and robust frailty model, where Patients are cared for on a virtual ward in their own home, ensuring their needs are met and they are monitored during an acute episode of illness. If you have existing nursing skills which you feel are not used to their full potential, and you feel your job is repetitive with the same tasks every day. Then look at this role, it provides a true variation of demand, it will make you utilise skills you maybe don't get to use. Every day you will look forward to coming to work, knowing you are preventing a hospital admission and supporting someone to remain at home.
Experience in community care is desirable, however full training can be provided, and ongoing post Reg training is available including the physical assessment course, diagnostics and independent nurse prescribing.

Essential
- A minimum of two years post registration experience.
- Experience of caseload management including responsibility for complex care packages for vulnerable people.
- Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting.
- Excellent communication and interpersonal skills.
- Broad range of enhanced clinical skills.
- Ability to advocate patient issues.
- Ability to demonstrate leadership skills.
- Demonstrates organisational skills: including the ability to make decisions and to prioritise.
- Demonstrates an understanding and insight into current issues and developments within the NHS.
- Involved in the implementation and management of change.
- Evidence of innovative practice.
- Experience of working with long-term conditions and frailty.

Qualifications
Essential
- BSc Nursing or RN currently registered with NMC.
- Post registration qualification or University Degree.
- Teaching and assessing qualification ENB 998 or equivalent / mentorship qualification - PETALS
- Management / Leadership Qualification / development programme.
- Ability to understand and interpret research findings/evidence-based care and apply to practice.
- Good knowledge of health and safety and risk management.
- Computer literate including emails and spreadsheets.
- Effective written and verbal communication skills.
Desirable
- Masters degree or equivalent experience gained by undertaking on-going personal development and training.
