Assistant Director Promoting Health & Wellbeing
The post-holder will contribute to the South Eastern HSC Trust’s health improvement agenda through the management of the Safe & Well Service which targets loneliness & reducing social isolation for older people. The specific health improvement focus of this post may change over time as service and population health needs dictate.
The post-holder will contribute to implementing in partnership, key local and regional strategies and priorities.
Working at both a strategic and operational management levels, he / she will provide leadership; support and capacity to ensure optimum local co-ordination and implementation of agree priorities and outcomes contributing to improving health & reducing health inequalities.
H/She will undertake line management responsibilities within the Health Improvement Team where appropriate and work comfortably and effectively within matrix and partnership structures to achieve agreed Health Improvement Outcomes. H/She will support the Head of Service and Health Improvement Team to meet and report on objectives required by South Eastern H&SC Trust performance management frameworks.
Provide highly specialist advice and planning support to Trust staff, other organisations and communities to improve health and wellbeing and to reduce inequalities in health
Lead on the development, implementation and monitoring of health improvement action plans in relation to a specific area/s to ensure the delivery of a quality service.
Work in collaboration with statutory, voluntary and community services to develop, implement, monitor and evaluate strategies, programmes and training in line with best practice to improve health and social wellbeing
Work in collaboration with statutory, voluntary and community organisations to sustain and develop initiatives and programmes to support older people who are lonely and or socially isolated
Develop, co-ordinate & monitor new programmes and initiatives in line with regional and local priorities.
Develop, deliver and evaluate internal and external organisations to support communities to contribute to improving health & wellbeing
Undertake as appropriate needs assessment in relation to health and social wellbeing
Ensure full engagement with Communities and Councils by creating & maintaining effective partnerships to effectively implement key health improvement strategies
Development, agree and monitor appropriate health improvement performance management targets in the service areas within his/ her remit of work using outcome based accountability methodology
Provide quarterly progress monitoring reports and end of year monitoring reports for the Public Health Agency.
Liaise with key stakeholders to determine the provision of training programmes, coordinate training programmes and identify gaps in provision
Coordinate and deliver training programmes and courses are required
Work closely with counterparts in the health development team and across all Trust directorates to ensure an integrated delivery of health improvement initiatives
Provide leadership and line management, as determined by service needs, for specific projects/services that promote the health and social wellbeing
Complete quarterly performance monitoring reports, evaluation reports and contribute to annual reports for the Health Development Department
Promote personal and public involvement in planning, development and implementation of health improvement activities
Maintain an up to date knowledge base of relevant research, policy and programmes relating to specific topic/s area of work to inform best practice;
Work in partnership with others within the Trust, other statutory agencies and community and voluntary sector groups and organisations to explore and maximise opportunities to attract additional funding and resource for the area
To carry out regular contract meetings with Providers to ensure agreed outcomes are being met and to put in place any actions to address areas of concerns.
Maintain a commitment to personal development, updating knowledge and skills and extending experience and expertise in areas relevant to the job remit;
To promote and lead on innovation and change to underpin the modernisation of health improvement / health promotion
Represent the interests of the Trust on regional & local partnerships such as Public Health Agency Physical Activity steering group, Local Groups & Council Community Planning Groups
