Hospital at Home (HAH) is a Consultant Geriatrician led multidisciplinary service within the South Eastern Trust, with the aim of providing hospital level care to frail older people living in the community. In order to meet the overall mission of the Trust the post holder will work in partnership with the HAH Operational Lead to:
Provide visible and effective leadership within the Hospital at Home team.
Takes day-to-day management responsibility for the planning and delivery of care in Hospital at Home.
Organise and coordinate team meetings.
Provide support and advice on specific nursing requirements.

Work as part of the multidisciplinary team.

Promotion of the Hospital at Home Service.

Maintaining effective communication between clients, carers, statutory and voluntary agencies and Primary Care.
Working with the Multi-Disciplinary teams in hospital and community as well as building strong links with Community and Voluntary sector organisations.
Strengthen links with Primary care.
Line management of the Nursing Team within Hospital at Home Service.
Reviewing clinical practices ensuring compliance with clinical governance structures, procedures and professional guidance.

Service Delivery
The Nurse Team Co-ordinator requires advanced communication and interpersonal skills to motivate, negotiate, and persuade internal and external stakeholders. The role requires clear, concise, and up to date communication to promote safe and effective care delivery.
The Nurse Team Coordinator will understand the patient needs and the specific requirements of those referred to the HAH service. They should be able to clearly articulate these needs and effectively delegate tasks to appropriate staff members, ensuring that care is delivered by those with the right skills.
Support the management of the performance of the Hospital at Home Service and ensuring robust processes are in place to support the delivery of high quality and safe care to meet all requisite standards.
Ensure there are robust access and care pathways to support the co-ordination of person centred care and integrated working.
Ensure the effective implementation of a case management approach with all key worker and team roles clearly understood.
Effectively manage the use of the nursing resource within the hospital at home through the completion of monthly off-duty rota, delegation of tasks on a day to day basis and oversight of bank staff usage.
Assist the HAH Operational Lead to liaise and work in partnership with hospital staff, GP’s and other relevant stakeholders to develop and implement the pathway for identification of clients suitable for Hospital at Home Service.
Ensure that nursing assessments are person-centred and completed in a timely manner.
To discuss with the multidisciplinary team the provision of treatment within the community setting in partnership with patients and carers, statutory and voluntary agencies.
Evaluate the outcomes of treatment programmes in achieving optimal outcomes for patient/carers.
Assist the HAH Operational Lead to evaluate service delivery against agreed key performance indicators and ensure that all staff are aware of monitoring process.
Ensure effective arrangements are in place for the identification, assessment and management of risks.
Manage the supervision of nursing staff within the Hospital at Home team.
Assist the HAH Operational Lead to ensure the processing and management of complaints, contract compliance, incidents and SAI comply with the Trust’s policy and procedures and are underpinned by transparency and a culture of continuous improvement.
Ensure effective implementation of the Trust’s Policy on Professional and Operational Management Interfaces within Hospital at Home Service.
To practice within the relevant Code of Professional Conduct and to maintain all team member practices under the NMC guidelines and regulatory body.
Keep abreast of changing government policies, targets and legislation relating to Health and Personal Social Services and national developments in relation to the care of older people, e.g. NSF for Older People, Transforming Your Care.
To network with appropriate professionals to share innovation, developing self and others in the area of Hospital at Home.
Participate in research, evaluation of projects, quality initiatives and developments and facilitate staff undertaking research and audit projects. To participate in any requirements under Clinical and Social Care Governance.
Participate in change management to meet the requirements of service provision and agreed strategies within the Reform, Modernisation and Efficiency Agenda.
Collate annual statistical reports and other information as requested by the Trust eg. Annual Evaluation Reports.
Keep accurate and contemporaneous records to comply with Trust standards, contribute to, and submit reports as requested for Hospital at Home Team.
Participate in the promotion of the Hospital at Home Team to senior managers, commissioners, GP’s and the general public.
Report complaints, near miss incidents and accidents, and undertake investigation and reporting relating to these incidents as required. Following investigation is responsible for implementation of action plans, monitoring and reviewing of outcomes.
Ensure the implementation of PPI and facilitate user/carer involvement in planning of future Hospital at Home Service.
Provide clear leadership and ensure the Hospital at Home Service have a highly skilled, flexible and motivated nursing workforce.
Ensure all people management practices within the Hospital at Home Service integrated and supports a culture of effective team working, continuous improvement and innovation.
Ensure the effective implementation of all Trust HR/People Management policies within the team and the achievement of all relevant targets such as those relating to the completion of personal development plans, management of sickness and absenteeism etc.
Ensure all staffing resources are deployed to maximum benefit.
Support the Assistant Director of Primary and Integrated Care, Operational manager and HAH Operational Lead with all workforce planning and reform initiatives.
Ensure the effective implementation of all Trust financial policies and procedures within the team.
Ensure the effective management, use and maintenance of all physical assets within each team.
