JOB SUMMARY
The Addictions Practitioner will work as a member of a multi-disciplinary team across the range of Health in Prison addiction service Liaise with community addiction services. They will ensure that a responsive high quality service is provided to service users, their families and carers. They will work within a matrix organisation as a member of the multi-disciplinary Health in Prison addiction team promoting effective inter-disciplinary and inter-agency working relationships and acting as a link between clients, professionals and other services to ensure continuity of care. They will be expected to work flexibly to ensure that the needs of the service are met in accordance with commissioned arrangements and key performance indicators.
The Addiction Practitioner will ensure the delivery of skilled, evidenced based interventions to individuals suffering from dependency on alcohol and/or other psychoactive substances. The Addiction Practitioner will contribute to the development of this dynamic learning environment by acting as and facilitating mentors for student nurses and others professionals placed with the team.
The post holder will be expected to hold a small caseload of opioid substitution clients as well as providing cover for colleagues and will be expected to deputise for or representing the Addiction Team Leader when required.

KEY DUTIES / RESPONSIBILITIES
Clinical/Professional/Specific Managerial Responsibilities
The post holder will
Work collaboratively with other members of the multi-disciplinary team in the delivery of mental health care services within Prisons
Act as a named keyworker for the management of a defined caseload the majority of which will be off moderate to severe complexity of need.
Undertake bio/psychosocial assessments, of routine & urgent cases, and formulation of care plans to meet the identified needs of the patient
Undertake Risk Assessments and formulate risk management plans as per Promoting Quality Care : Good Practice Guidance on the Assessment and Management of Risk (2009)
Review and evaluate care plans and risk management plans on an ongoing basis
Regular attendance and participation at the multi-disciplinary clinical meetings and daily huddle that facilitates discussion of patient/client needs and ensures a shared approach to care, treatment and risk management
Liaise and work collaboratively with CMHTs and Forensic Mental Health Services when necessary ensuring the safe transfer and/or discharge of patients
For Registered Nurses only: Administration of medications including Controlled Drugs (CDs) and ensure the maintenance of the Controlled Drug registers and the Controlled Drug Cabinet
Delivery of evidence based psychological and psychosocial interventions as appropriate to the needs of the patient/client, including participation in group programmes
Promote person centred care and the recovery approach in the delivery of all clinical interventions
Providing psycho-education and health promotion advice to enhance the mental health and well-being of patients/clients and that promotes self-management
Advice regarding medication management including promoting concordance and monitoring and advising regards effect and side effects
Provide response to emergency/urgent care as required and ensure appropriate onward referral and care arrangements are made as required
Contribute to the Supporting People at Risk (SPAR) process including participation in the SPAR reviews and sharing relevant information to assist effective risk assessment and management plans
Monitor and report on patients/clients progress and maintain contemporaneous records, ensuring all clinical information is entered onto the patients’ computerised medical records (EMIS)

Develop personal knowledge and skills to level 2-4 of the SBNI ‘Child Safeguarding Learning and Development Strategy and Framework’ (2020-2023).

To act as a role model to promote a culture of research and reflective practice within the ward / department to enhance person-centred care.
Ensure evidence-based care is provided to agreed standards within the ward / department.
Assist in developing, implementing and monitoring policies, procedures and protocols.
Create an environment which supports a culture of learning and reflective practice for all staff.
Assist the team leadership to ensure that systems and processes are in place to support effective mentoring of relevant staff.
Environment
To maintain a safe and clean environment for staff, patients / clients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection, risk management and critical incident reporting.
Ensure staff awareness of environmental issues and take appropriate action as per HSC Trust policy.
Assist the team leadership with the analysis, assessment and management of actual and potential risks to health and well-being.

Ensure near misses, incidents, accidents and faulty devices are recorded, reported, investigated and learning disseminated as per HSC Trust policy.

Contribute to the establishment of systems and processes to ensure effective communication and continuity of patient / client care, liaising with multi-disciplinary / multi-agency teams, community services and colleagues within the Northern Ireland Prison Service.
Contribute to the Patient Safety and Support and Supporting People at Risk (SPAR) processes, sharing relevant information to assist effective risk assessment and management plans
