CDDFT community services are offering an exciting opportunity for a registered nurse to join the team as a 9-month secondment opportunity covering maternity leave based in Bishop Auckland supporting patients living across Durham Dales, Sedgefield and Darlington suffering a variety of respiratory conditions including COPD, Emphysema, Asthma, Bronchiectasis and Pulmonary Fibrosis.
The post is part time, 24.5 hours a week. The service is open Monday-Friday 9am 5pm, hours can be negotiated and flexible over 3 or 4 days.
Main duties of the job
- Manage a caseload of patients suffering a respiratory condition as part of the wider team supported by Respiratory Specialist Nurses.
- Patient education supporting self-care and management including proactive exacerbation support, pulmonary rehabilitation and smoking cessation.
- Medication optimisation and inhaler technique.
- Patient triage
- Support patient support groups, lower band staff members plus students.
- Contribute to creating improvement, transformation and innovation within the service to promote best practice.
- Demonstrate effective time management skills.
- Commit to continuous personal development/lifelong learning by attending mandatory training, use of eLearning and adhere to all trust policies and procedures.
- To understand owns role within the wider multi-disciplinary team and articulate own developmental needs through attending clinical supervision, 1:1 sessions, undertaking appraisal preparation and contributing to objective setting and personal development planning.
- Promote service development, improvements and quality expansion and efficiencies.
- Support provision for a high quality, value for money and effective service.
- Effective use of IT systems to ensure accurate data recording.

Job responsibilities
Clinical Responsibilities
- To co-manage a case load of complex respiratory patients, under direction of the community respiratory nursing team, across the community respiratory specialist service, within clinic and patients home environments, Monday-Friday 9am-5pm.
- To undertake holistic patient respiratory assessment and review including social and psychological well-being and progress treatment interventions and adapt interventions, within set protocols, to progress treatment interventions and to meet the changing needs of the patient.
- Develop, implement and evaluate patient treatment plans.
- Communicate patient treatment plan changes to respiratory specialist nursing team.
- Understand basic respiratory anatomy and pathophysiology and the function and management of all respiratory conditions including mental well-being.
- Diagnostic spirometry performance qualification and registration either through The Association for Respiratory Technology and Physiology (ARTP) or at certified ARTP standard, is desirable but not essential as training will be provided.
- Perform capillary blood gas testing and oxygen reviews.
- Provide long term condition care following the COPD pathway.
- Accurately record clinical data and understand its significance including heart rate and rhythm, respiratory rate, pulse oximetry, peak flow rate, and blood pressure.
- Complete effective record keeping which demonstrates the decision-making process, care planned and given, as well as patient outcomes in accordance with trust policy and legal requirements.
- Understand the benefits of non-pharmacological interventions for respiratory illness/conditions including smoking cessation, weight management and nutrition, physical activity and brief cognitive behavioral therapy (CBT).
- Understand motivational skills and empathetic communication.
- Promote health and healthy informed choices.
- Review smoking status, provide very brief advice and refer to appropriate smoking service.
- Have knowledge of the benefits of exercise and pulmonary rehabilitation and refer to appropriate local rehabilitation programs.
- Review and train inhaler techniques to patients, carers and other health care professionals.
- Participate in service improvement and development initiatives.
- Have awareness, knowledge and implementation of safeguarding processes in relation to children, vulnerable patients and carers.
