Band 7 Renal Transplant Specialist Nurse - Permanent - Full time

A new exciting opportunity has risen, to provide expert nursing coordination of care for patients during their kidney transplant journey. The post holder would be responsible for managing their own case workload, working autonomously as well as the Trust and regional MDT. The role would consist of leading and facilitating clinical preparation of living donors of both recipients and donors in North Cumbria.

The role requires specialist Nursing expertise, knowledge and skills in the clinical practice of Transplantation Nursing Care, working closely alongside the CKD Nursing team and wider renal services. The role also requires education to be enhanced through the teaching of patients, families and colleagues, in order to enhance clinical practice. The post holder will possess clinical skills in order to carry out clinical assessments, investigations and tests.

The Renal service at NCIC has a cohort of 2,300 patients, of whom 171 are transplant recipients.  It comprises of two Renal Units, with an acute Renal Unit at CIC site and a satellite Renal Unit at the WCH, a 14 bedded Renal ward, CKD team and a Home Therapy team as well as a dedicated team of Nephrologists.

Main duties of the job

  • Organise and facilitate living donor transplant clinical assessments, investigations and tests required up to the point of Consultant Nephrologist appointment, utilising existing pathways initially.
  • Work alongside Consultant Nephrologists and CKD nurse specialists regarding referrals for transplant recipients
  • Co-chair the renal transplant MDT meetings with Renal Transplant Lead Nephrologist
  • Attend relevant regional meetings e.g. Living donor radiology meeting, regional renal transplant executive meeting; to facilitate transplant listing and donation
  • Work alongside patient’s parent Nephrologist and  CKD nurses to ensure early transplant care and follow up is provided in line with regional guidelines
  • To lead on all nursing aspects of early transplant care alongside parent nephrologist
  • Provide patient education regarding kidney transplant work up and care
  • To raise the profile of living kidney donation in North Cumbria and encourage increased take up of living donation
  • Demonstrate excellent communication skills and provide emotional support to patients and relatives surrounding the sensitive subject of Transplant whilst portraying empathy.
  • Provide excellent clinical leadership and management skills to ensure the effective delivery of patient focused care and resources within the remit of the department and within the parameters of agreed Trust policies and guidelines.
