Job summary

Join our dedicated multidisciplinary team within the Barking and Dagenham, Havering, Redbridge Falls Service, based in the BHR Borough. This role focuses on providing patient-centred falls management and support to adults with diverse physical, cognitive, and social care needs. This position offers an opportunity to make a meaningful impact by promoting independence and enabling individuals to remain safely in their homes or communities.

Main duties of the job

As a member of the BHR Falls Service, you will play a key role in supporting adults with a wide range of needs through tailored therapy programmes. Working under the direct supervision of qualified therapists, you will help maximise patients' independence, improve their functional ability, and support their safety at home or in the community. This is a dynamic and rewarding role that requires a commitment to promoting dignity, safeguarding, and person-centred care. Deliver patient-centred falls management programmes in group settings and one-to-one sessions under the guidance of a qualified therapist. Monitor patient progress, provide feedback to therapists, and identify when reassessment or additional support is required. Manage a delegated caseload, implementing, reviewing, and adapting treatment plans as needed. Undertake home visits to review exercise programmes, provide falls prevention education, and support safe, independent living. Collaborate with patients, families, carers, and the multidisciplinary team to contribute to care planning and decision-making. Maintain accurate clinical records and adhere to legal, professional, and safeguarding frameworks, including the Mental Capacity Act and infection control policies. Support service improvements, maintain equipment inventory, and ensure the effective use of resources. Promote high standards of patient care in line with local and national policies and Trust procedures.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
